This form serves as the official online platform for:
• Course Inclusion Requests
• Section Requests
• Open Section Petitions

It will also serve as the Online Student-Faculty Appointment (SFA) for enrollment concerns regarding unavailable course sections.

⏳ Please note that your concern may be accommodated starting tomorrow.

❗There will be no more onsite SFA. All SFA concerns must be submitted through the Course Petition Form.

Exception: Students from programs other than Civil Engineering may still proceed to the faculty office, provided that their Program Chair is available.

📢 𝐀𝐍𝐍𝐎𝐔𝐍𝐂𝐄𝐌𝐄𝐍𝐓 | 𝐄𝐍𝐑𝐎𝐋𝐋𝐌𝐄𝐍𝐓 𝐔𝐏𝐃𝐀𝐓𝐄𝐒

Starting August 5, all SFA transactions will be conducted online through the 𝗖𝗼𝘂𝗿𝘀𝗲 𝗣𝗲𝘁𝗶𝘁𝗶𝗼𝗻 𝗙𝗼𝗿𝗺. The Registrar's Office (RO) will continue to be available at the SCEGE Faculty Office throughout the week to assist students with registrar-related concerns.

✅ Use the form to submit concerns regarding:
• Course Inclusion
• Course Petition Requests
• Other SFA-related concerns

📌 If you already answered the Course Demand Form last Monday or Tuesday (August 3 & 4), there is no need to submit another response. The Course Petition Form is simply the updated name of the same form, and duplicate submissions may affect the processing of requests.

All residency students are required to submit a Waiver Form for the CE Exit Examination to be included in the official list of examinees for this term.

Please note the following:

- The deadline for Waiver Form submission is on August 7, 2026 (Friday).
- Prepare two (2) copies of the Waiver Form with identical contents.
- Even if you have not yet officially filed for residency, you must still submit the Waiver Form.

Furthermore, there will no longer be a need to message Ma'am Sulit during the second week of September to file for residency. The submitted Waiver Form will also serve as your Residency Application and billing request.

Please ensure that you submit your forms on or before Friday to avoid any delays in your residency processing and inclusion in the CE Exit Examination list.
