What the CCTN Really Means

Today, the four letters CCTN mean something very different to me.

They are no longer about recognition.

They are about responsibility.

Every toxicology nurse who earns the CCTN carries a unique story.

Some have spent countless nights caring for critically poisoned patients.

Some have dedicated themselves to teaching.

Others have advanced research, policy, or advocacy.

The certification does not create those stories.

It simply recognizes them.

As for my own CCTN, I have come to realize that its greatest value was never that it was conferred by an organization.

Its greatest value comes from the trust placed in me by my colleagues, mentors, students, and fellow toxicology nurses.

That trust is a far greater credential than four letters.

To every present and future CCTN:

Wear those letters with humility.

Continue learning.

Remain accountable.

Never forget that every credential carries a responsibility to serve society.

In the end, the true meaning of CCTN is not found after our names.

It is found in the lives entrusted to our care.

Science. Compassion. Justice.

The Burden of Being First

There is something uniquely uncomfortable about helping create a credential that you yourself will eventually bear.

How do you avoid serving yourself?

How do you ensure that your decisions benefit the profession rather than your own career?

Those questions weighed heavily on me.

When TNAP was founded, I was the youngest and among the least experienced person in the room.

My greatest contribution was not that I knew the most.

It was simply that I was willing to work.

Someone had to carry the burden of beginning.
Someone had to draft the competencies.
Someone had to write the documents.
Someone had to take the first step.

Looking back, I realize that pioneering is rarely glamorous.

It is often uncertain, uncomfortable, and filled with self-doubt.

But every profession needs people willing to begin.

Becoming the Experts We Could Not Find

One of the greatest challenges was simple.

We had no experts to guide us.

If we wanted competencies, we had to learn how competencies are developed.

If we wanted standards, we had to study how standards are written.

If we wanted certification, we had to understand how certification should be designed.

Most of what we learned came through trial and error.

There were mentors who helped us, for which I remain deeply grateful.

But much of the journey required us to teach ourselves.

People sometimes call me an expert in toxicology nursing.

The truth is, I have never been comfortable with that description.

I remain a student.

I continue to be corrected.

I continue to be challenged.

And I hope I never lose that humility.

Perhaps expertise is not arriving at the destination.

Perhaps it is remaining committed to learning for the rest of your life.

Part II

Building Something That Did Not Yet Exist

One of my colleauges, Sir John Bernardo, would often remind us:

“Dapat kinikilala tayo ng PRC-BON.”

Those words stayed with me.

Creating a specialty certification was never simply about producing another certificate.

It meant defining competencies, setting no standards, protecting the public, and carrying the responsibility of representing an entire specialty.

At that time, I honestly believed we were not ready.

We lacked established competency frameworks.

We had very few local references.

We had limited experience.

Most importantly, I questioned whether we had earned the right to create something that would represent every toxicology nurse in the country.

So I waited.

For more than a year, I wrestled with that decision.

Then I realized something.

If we kept waiting for someone else to build the foundation, perhaps no one ever would.

Sometimes, pioneering means beginning before everything is perfectly in place.

Not recklessly.

But courageously.

Part I

The Four Letters I Never Wanted

When people see CCTN after my name, many assume it was something I always wanted.

The truth to be told, it was a half-truth. Yes, I wanted it but in a different way. And I learned it the hard way.

I was probably one of the most hesitant people to create it.

Not because I doubted toxicology nursing.

I questioned whether we needed another set of letters attached to our names.

As nurses, we celebrate becoming RN because it signifies our entry into one of the noblest professions. Yet somewhere along our careers, we begin collecting credentials, certifications, fellowships, and post-nominal titles. In today’s merit-driven world, those letters often become symbols of prestige.

I understand that feeling.

When I was a nursing student, I carried a small piece of paper with my name followed by every credential I dreamed of earning one day. Looking back, it was wishful thinking, youthful ambition, and perhaps a search for identity.

But years later, I found myself asking a different question.

What are those letters really for?

Should they elevate the individual?

Or should they serve the profession?

That question became the beginning of my journey with the CCTN.
