🛎️ NEW STUDENT APPLICANTS PROCESS FLOW:
🔴 STEP 1:
Present the required documents (highlighted in yellow) to the Student Affairs and Services (SAS) Office.
🔴 STEP 2:
Pay ₱200.00 at the Cashier’s Office for the Entrance Examination Fee.
🔴 STEP 3:
Bring the official receipt to the Guidance Office and schedule your Entrance Exam.
🗓️ Be sure to come on your scheduled exam day!
🔴 STEP 4:
Wait for the posting of the Entrance Exam Passers. 📝
📄 GENERAL REQUIREMENTS
✅ For Senior High School Graduates: ALL ORIGINAL
1 - Form 138 or Senior High School Report Card
➡️ Duly signed by the Principal (no facsimiles allowed)
➡️ Marked "Graduated" or "Promoted to College"
2 - PSA/Live Birth Certificate
3 - Certificate of Good Moral Conduct from Previous School
4 - Certificate of Barangay Residency, Indigency, and Voter’s Registration
✅ For Transferee Students: ALL ORIGINAL
1 - Honorable Dismissal with Transcript of Records
2 - PSA/Live Birth Certificate
3 - Certificate of Good Moral Conduct from Previous School
4 - Certificate of Barangay Residency, Indigency, and Voter’s Registration
✅ Additional Requirements upon Passing the Entrance Exam:
* Medical Certificate with Laboratory Results (X-ray, Urinalysis, Drug Test, CBC)
* 2x2 Pictures (6 copies, white background)
* Long Expanded Brown Envelope
* PSA Marriage Certificate (for married females)
