✅ For males, shave your mustache
• Proper haircut (No lining)
✅ For females, bun/hairnet

Students who are planning to enlist in NSTP-ROTC are advised to submit their completed enlistment forms to the NSTP/DMST Office on or before the extended deadline.

𝗣𝗹𝗲𝗮𝘀𝗲 𝗲𝗻𝘀𝘂𝗿𝗲 𝘁𝗵𝗮𝘁 𝘁𝗵𝗲 𝗳𝗼𝗹𝗹𝗼𝘄𝗶𝗻𝗴 𝗶𝘀 𝗮𝘁𝘁𝗮𝗰𝗵𝗲𝗱 𝘁𝗼 𝘆𝗼𝘂𝗿 𝗲𝗻𝗹𝗶𝘀𝘁𝗺𝗲𝗻𝘁 𝗳𝗼𝗿𝗺:
• Photocopy of the valid ID of your parent or guardian
• Three (3) specimen signatures of your parent or guardian
