Annual Financial Assistance Pay out for Dona Salad Chapter members only whose ages from 65 up will be on May 5,2023 start at 8am at Dona Salud Subd. Gym.
What to bring:
1. Xerox copy of OSCA id with 3 signatures
2. If authorized. Representative bring the zero copy of OSCA id of the beneficiary with 3 signatures , plus authorization letter, plus xerox copy of representative id with 3 signatures plus claim stub.
Claim stubs will be given tomorrow May 4 at the senior citizens office Purok 6 Carvajal residence.
Take note: 1.Use face mask
2. Bring your own ball.
