Bring the following requirements on your college admission test schedule:
1. Printed Admission Form (please print it in long bond paper)
2. 2x2 ID picture (white background with nametag)
3. Certification from the School Principal that applicant is a graduating SHS student (for incoming first year students / Certification of Grades showing all the subjects taken from school last attended (for transferees)
4. Ball pen
5. 1 Clean sheet of bond paper

Submit the following requirements:
FOR INCOMING FIRST YEAR STUDENTS
1. Report Card (Form 138);
2. Certificate of Good Moral Character;
3. Photocopy of Senior High School Diploma;
4. Four (4) Copies of 2x2 ID Picture;
5. Authenticated Copy of PSA/NSO Birth Certificate; and
6. Other requirements prescribed by the College/Department, CHED, PRC as deemed necessary.
FOR TRANSFEREES
1. Certification of Grades showing all the subjects taken from school last attended;
2. Honorable Dismissal;
3. Certificate of Good Moral Character;
4. Four (4) copies of 2x2 ID picture with white background and name tag (scanned picture or digital copy not accepted);
5. Authenticated Copy of PSA of Birth Certificate;
6. Accomplished Substitution Form of subjects taken from other SUCs as deemed necessary; and
7. Accomplished Validation Form of subjects taken from non SUCs
Additional requirements for BSCrim, BSLEA & BSISM
1. Hepatitis B Test Result
2. Chest X-Ray Result
3. Drug Test Result
Additional requirements for BSHM & BSTM
1. Hepatitis A and B Test Result
2. Chest X-Ray Result
