- Completion of the Profiling Form (by class)

- 1 Folder per Faith-based Organization (with CBL, name of organization, updated membership fee, activity for the whole year, name and contact numbers of officers)
* per Organization should consist of atleast 15 officially enrolled students in the campus
*Catholics should create 1 parochial unit per college with 2 representatives per department in the officers
