WHEREAS, Section 384 of Republic Act No. 7160, otherwise known as the Local Government Code of 1991, declares that the Barangay serves as the primary planning and implementing unit of government policies, plans, programs, projects, and activities in the community and as a forum wherein the collective views of the people may be expressed, crystallized, and considered;

WHEREAS, Section 391(a)(1), (2), and (3) of Republic Act No. 7160 empowers the Sangguniang Barangay to enact measures, approve resolutions, and provide services and facilities necessary to promote the general welfare and improve the quality of life of the inhabitants of the Barangay;

WHEREAS, Section 394(d)(1), (2), and (8) of Republic Act No. 7160 provides that the Barangay Secretary shall keep custody of Barangay records, prepare and maintain official documents and perform such other duties and functions as may be prescribed by law, ordinance, or resolution;

WHEREAS, Section 389(b)(1), (3), and (5) of Republic Act No. 7160 mandates the Punong Barangay to enforce laws and ordinances, administer the operation of the Barangay government, and ensure the delivery of basic services and facilities to the residents of the Barangay;

WHEREAS, a significant number of residents of Barangay [BARANGAY NAME] belong to economically disadvantaged and low-income households who regularly seek financial assistance from various government agencies and offices, including but not limited to the Department of Social Welfare and Development (DSWD), Provincial Government, Municipal Government, Office of the District Representative, Philippine Charity Sweepstakes Office (PCSO), PhilHealth, and other public and private charitable institutions;

WHEREAS, requiring indigent applicants to secure notarization or certification from distant offices often results in unnecessary expenses, inconvenience, and delays that adversely affect the timely release of urgently needed assistance;

WHEREAS, the Barangay Government, through the Punong Barangay and Barangay Secretary, possesses the authority and capability to verify the authenticity of documents presented by constituents by comparing photocopies against the original documents and maintaining official records thereof;

WHEREAS, the participation of the Chairperson of the Committee on Health, Social Services, and Family Welfare of the Sangguniang Barangay in the verification process shall strengthen transparency, accountability, and integrity in the issuance of certifications;

WHEREAS, the establishment of an official verification and documentation procedure is consistent with the principles of accountability, transparency, public service, and proper record management required under Republic Act No. 6713, the Code of Conduct and Ethical Standards for Public Officials and Employees, and applicable rules of the Commission on Audit (COA), Department of the Interior and Local Government (DILG), and Civil Service Commission (CSC);

NOW, THEREFORE, upon motion of Kagawad ______________________________, duly seconded by Kagawad ______________________________, be it:

RESOLVED,

d. Medical Certificates;
e. Clinical Abstracts;
f. Laboratory and Diagnostic Results;

i. Funeral Invoices and Receipts;
j. Death-Related Medical Documents;
k. Other supporting documents required for medical, hospitalization, burial, educational, or social welfare assistance.

SECTION 2. CONDITIONS FOR ISSUANCE

Certification shall be issued only upon compliance with the following requirements:
a. Presentation of the original document for direct examination and comparison;
b. Submission of photocopies corresponding to the original documents presented;
c. Presentation of a valid government-issued or company-issued identification card;
d. Submission of proof of residency within Barangay [BARANGAY NAME];
e. Submission of a valid Barangay Certificate of Indigency, whenever required;
f. Personal appearance of the applicant, authorized representative, or immediate family member;
g. Recording of the transaction in the Official Barangay Certification Logbook.

SECTION 3. VERIFICATION PROCEDURE

Prior to certification, the following verification procedure shall be observed:
a. The Barangay Secretary shall examine and compare the original document against the photocopy submitted;
b. The Chairperson of the Committee on Health, Social Services, and Family Welfare shall verify the residency and indigency status of the applicant whenever necessary;
c. The Punong Barangay shall review and approve the certification after determining compliance with the requirements of this Resolution;
d. The certification shall contain a statement indicating that the photocopy was compared with the original document presented.

SECTION 4. OFFICIAL LOGBOOK

The Barangay Secretary shall maintain an Official Certification Logbook containing the following information:

β€’ Date of Issuance;
β€’ Name of Applicant;
β€’ Address;
β€’ Type of Document Certified;
β€’ Purpose of Certification;
β€’ Number of Pages Certified;
β€’ Signature of Applicant;

SECTION 5. VALIDITY AND USE

Certified true copies issued pursuant to this Resolution shall be used exclusively for applications for financial, medical, hospitalization, burial, educational, and other social welfare assistance programs administered by government agencies and instrumentalities.

These include, but are not limited to:
β€’ Department of Social Welfare and Development (DSWD);
β€’ Municipal Social Welfare and Development Office (MSWDO);
β€’ Provincial Social Welfare and Development Office (PSWDO);
β€’ Office of the District Representative;

β€’ PhilHealth;
β€’ Local Government Units;
β€’ Other government and charitable institutions.

SECTION 6. LIMITATIONS

This authority shall not apply to documents which by law must be certified exclusively by the issuing office or agency, including but not limited to:
a. PSA and Local Civil Registry Birth Certificates;
b. Marriage Certificates;
c. Death Certificates;
d. Court Orders and Judicial Records;
e. Land Titles and Certificates of Title;
f. Tax Declarations;
g. Official Government Licenses and Permits;
h. Other public records required by law to be certified only by the originating agency.

SECTION 7. ACCOUNTABILITY

Any Barangay official or employee who knowingly certifies falsified, altered, fraudulent, or spurious documents shall be held administratively, civilly, and criminally liable under applicable laws, including:
β€’ Republic Act No. 7160;
β€’ Republic Act No. 6713;
β€’ The Revised Penal Code provisions on Falsification of Public Documents;
β€’ Applicable rules of the Commission on Audit (COA);
β€’ Civil Service Commission regulations;
β€’ Department of the Interior and Local Government issuances.

SECTION 8. SEPARABILITY CLAUSE

If any provision of this Resolution is declared unconstitutional or invalid, the remaining provisions not affected thereby shall continue to be in full force and effect.

SECTION 9. REPEALING CLAUSE

All Barangay resolutions, orders, rules, regulations, or portions thereof inconsistent with the provisions of this Resolution are hereby repealed, amended, or modified accordingly.

SECTION 10. EFFECTIVITY

This Resolution shall take effect immediately upon approval.
