The Hub and the Free Public Advocacy Service (FPAS) have developed a Community Will Pack to help people record their wishes in a practical and accessible way.

The pack includes:
• guidance pages,
• the Community Will Template,
• an optional Organ & Tissue Donation Codicil,
• a plain-language glossary explaining some of the terms used,
• and an “Other Important Things to Think About” page.

Full information is available on our website 👉www.freepublicadvocacyservice.co.nz/community-will-pack

The Hub / FPAS act as access facilitators and can assist with practical questions about how the process works, but cannot provide legal advice.

If your situation is more complex, or if you feel unsure at any stage, legal advice is recommended.
