Who is the father of the little girl? How did the patient get pregnant?
A 43-year-old patient, female, was reported to TPO Cambodia team by her older sister, requesting treatment for a serious mental illness that has gotten her sister confined and chained up in her home. When the team arrived, the patient initially appeared calm. She put on a friendly face and kindly welcomed our team. She was sitting on a hammock while eating mangos. Her hands were filled with mangos, while her leg was chained. In the meantime, her 5-year-old daughter started crying, the patient suddenly became aggressive. No one knew how the patient was feeling, perhaps she thought someone wanted to hurt her daughter. Even though she lost her insight, she was still protecting her child, afraid that her daughter would be hurt. According to her older sister, the patient began to fall ill in 2017, and she was sent back to Cambodia while working as a house cleaner in Malaysia, a job she had held for over a year. We had no information on what happened to her during her work in Malaysia. The patient’s family took the patient to a few public hospitals in Phnom Penh for treatment, but there were no signs of improvement. Due to a lack of alternatives, financial problems, and safety concerns, the patient’s family decided to lock her up in her house.
At that point in time, the COVID-19 pandemic was beginning to spread widely. The patient escaped her house by breaking down the window. Three days later, the patient returned home and was chained and locked up by her family. A few months later, her relatives noticed that the patient was experiencing symptoms of morning sickness, but they brushed it aside. Later, when she was 5 months pregnant, there was still nobody who noticed and the baby wasn’t moving typically. When the baby was 7 months old, the relatives realized that the patient was pregnant. Fortunately, since pregnancy, the patient behaved and appeared calm, which is called that baby brings luck. The patient was in time for the birth of her child during the Water Festival while COVID-19 was still spreading. At that time, the patient was taken care of by her elderly parents. Her elderly mother went to the health center, looking for an available midwife but no midwife was willing to help her give birth. The mother ran to a traditional birth attendant, but she also refused because she was afraid of COVID-19 and was also afraid of her safety because it is known that the pregnant woman is psychotic. With no alternatives, the elderly mother prepared a scissor and helped deliver the baby herself. If you form a mental image of the situation, it is a very unfortunate and tragic moment. Her life seems more miserable than the life of the Khmer legend Badacar. After giving birth, her relatives took the baby to the health center to check the baby's health. The baby was healthy with no health problems. There were no prenatal checkups, no prenatal care, no vaccinations, and she had to give birth without a professional midwife. It is truly a blessing that the baby is healthy and normal.
During the assessment interview, the patient talked to herself, could not communicate with others and had almost no insight but she still recognized her child. The patient received medication and the therapists provided psycho-education to her family so that they could understand the disease and effective treatment methods. TPO Cambodia needs more support and financial assistance from generous individuals to provide treatment to many more mentally ill people who are still confined.
