NFHS-6 shows encouraging progress: more young children are receiving an adequate diet, more mothers are taking iron-folic acid supplements, and some child growth indicators are improving.

At the same time, many children are still not getting the nutrition they need, and anaemia data is still awaited through DABS-I. (Diet and Biomarkers Survey in India)

What does impact look like when no one knows your name?
For most of the people Fortify Health serves, the answer is simple: a bag of wheat flour that contains more nutrition than it otherwise would have.

They may never hear of Fortify Health. Yet every month, millions of people, including children and pregnant women, consume fortified flour produced through partnerships with millers across India.

This week, Elie Hassenfeld (CEO, GiveWell) and Andrew Martin (Senior Research Analyst, GiveWell) released a podcast reflecting on a decade of GiveWell's work funding interventions to address iron deficiency anaemia globally, including a discussion of Fortify Health's work in India.

The conversation explores what it takes to scale a public health intervention, how evidence shapes decision-making, and why food fortification continues to be one of the most promising ways to improve nutrition at the population scale.

Most public health interventions fail not because the science is wrong, but because nobody designed them for the people who actually have to do them and pay for them.

Food fortification is different. A landmark Lancet study published this year (Friesen et al., 2026) confirms it works, it's cost-effective, and the scale of what remains unachieved is enormous.

But knowing it works and knowing how to make it scale are two entirely different things. Two questions determine everything:

Who is actually going to do this at scale?
And who is going to pay for it?

The answer shapes every design decision that follows, and it looks completely different depending on whether the answer is a government, a business, or both.
