1. अस्पताल 24×7 चलता है, होटल नहीं।
अस्पताल में डॉक्टर, नर्सिंग स्टाफ, टेक्नीशियन, वार्ड बॉय, सफाईकर्मी, सुरक्षा और प्रशासनिक स्टाफ की व्यवस्था चौबीसों घंटे रखनी पड़ती है—चाहे उस समय बेड खाली ही क्यों न हो।
2. कमरे के साथ चिकित्सा सुविधा भी उपलब्ध रहती है।
अस्पताल के बेड के आसपास ऑक्सीजन, सक्शन, मॉनिटरिंग, नर्स-कॉल सिस्टम, इमरजेंसी उपकरण आदि की व्यवस्था होती है। सरकारी Clinical Establishment standards भी अस्पतालों में पर्याप्त चिकित्सा उपकरण और उनका नियमित रखरखाव अपेक्षित करते हैं।
3. अस्पताल को हमेशा आपातकाल के लिए तैयार रहना पड़ता है।
मरीज को अचानक cardiac arrest, bleeding, respiratory failure या अन्य emergency हो सकती है। इसलिए crash cart, defibrillator, oxygen, emergency drugs और प्रशिक्षित स्टाफ हमेशा तैयार रखना पड़ता है।
4. ऑपरेशन थिएटर और ICU जैसी सुविधाओं का खर्च बहुत अलग है।
Sterilization, OT equipment, anesthesia equipment, ICU monitoring, ventilator, biomedical equipment और उनके maintenance का खर्च सामान्य कमरे के किराये में दिखाई नहीं देता, लेकिन अस्पताल को वह लगातार वहन करना पड़ता है।
5. अस्पताल में महंगे उपकरण 24 घंटे उपलब्ध रखने पड़ते हैं।
मशीन का उपयोग दिन में कुछ घंटे हो सकता है, लेकिन उसकी खरीद, AMC, calibration, electricity और maintenance का खर्च लगातार चलता रहता है।
6. बिजली और ऑक्सीजन की व्यवस्था होटल से बिल्कुल अलग है।
अस्पताल में backup power, oxygen supply, suction, critical equipment और कई जगह HVAC/air-handling systems की आवश्यकता होती है। बिजली जाने पर मरीज की चिकित्सा व्यवस्था बंद नहीं की जा सकती।
7. साफ-सफाई और infection control का खर्च अधिक होता है।
अस्पताल में सामान्य housekeeping के अलावा sterilization, infection-control protocols, biomedical waste segregation/disposal, PPE और विशेष disinfectants की आवश्यकता होती है। Clinical Establishments Act का उद्देश्य ही न्यूनतम सुविधाओं और सेवाओं के मानक निर्धारित करना है।
8. अस्पताल खाली बेड की लागत भी उठाता है।
किसी होटल का कमरा खाली है तो उसे बंद किया जा सकता है। अस्पताल में emergency के लिए बेड, स्टाफ और उपकरण उपलब्ध रखने पड़ते हैं। इसका खर्च केवल भर्ती मरीज से ही निकलता है।
9. निजी अस्पताल को अपनी पूरी infrastructure cost खुद निकालनी होती है।
भवन, medical equipment, licenses, fire-safety व्यवस्था, बिजली, पानी, maintenance, कर्मचारी वेतन, insurance, IT/HMIS, laboratory, pharmacy और अन्य operational खर्च लगातार होते हैं।
10. सही तुलना केवल कमरे की कीमत से नहीं, पूरी सेवा से होनी चाहिए।
होटल ₹3,000 का कमरा देता है; अस्पताल उसी राशि में केवल सोने की जगह नहीं देता—वह चिकित्सा निगरानी और तत्काल उपचार उपलब्ध रखने वाली healthcare infrastructure का हिस्सा देता है।

इसके अलावा समय-समय पर विभिन्न राज्य सरकारे अस्पतालों पर पेनाल्टी लगाती हैं तो कुछ लोग खुशी से पागल हो जाते हैं क्योंकि उन्हें यह बात पता ही नहीं होता कि इस पेनाल्टी का खर्च भी भविष्य के मरीजो के बिल में ही जुड़ेगा।

एक अस्पताल खोलने के लिए लगभग 54 किस्म के लाइसेंस लेने पड़ते हैं।

यदि कोई चैरिटी हॉस्पिटल खोलना चाहे तो उनके लिए यह यूजफुल जानकारी डिलीवर की जा रही है।

हमारे देश में 37 लाख NGO है। यदि सभी एक-एक बड़े कॉर्पोरेट फाइव स्टार लग्जरी चैरिटेबल अस्पताल खोल लें तो आम जनता उन्हें बहुत आशीर्वाद देगी।

अस्पताल के खर्चों के बारे में सही-सही जानकारी न होने के कारण ये एन जी ओ अभी तक चैरिटेबल अस्पताल नहीं खोल पा रहे थे।

ARTIFICIAL INTELLIGENCE AND LONELINESS : REGULATION NEEDED
Artificial Intelligence has witnessed a proliferation of chat bots as AI companions for a fee. These social chatbots provide " happy conversations " and a seemingly perfect relationship at the cost of mutual vulnerability and genuine belonging we gain from human bonding.
For those experiencing social disconnection, chatbots alleviate symptoms of loneliness, chronic disconnection and thwarted belonging. But, the risk of replacing human relationships with chatbots are only starting to be understood.
Initially, chatbots were used by students for academic support besides loneliness. Currently, the risk of social chatbots, such as dependency, displacing human relationships, withdrawing socially or building unrealistic relationships outweigh the benefits.
New rules are needed to regulate AI companies and chatbots. Alongside chatbots must gaurd state secrets, restricting emotional use of these tools and following social and moral standards. Material harm from using companion AI do occur.
China has enacted measures for social AI bot developers to assure that the content does not -
* threat National security or National honour.
* violate core social values
* encourage self harm or su***de
* excessively caters to emotional dependency or addiction
* Provide " virtual partners " or relatives to minors
* serve unsuitable content to minors.
Though it is not healthy for young people to rely exclusively on AI as a trusted friend, Yet conversational use of AI alleviate the feelings of loneliness. The reliance could lead to an AI induced " loneliness spiral " because chatbots can actually exacerbate it. Restrictions can help break to this cycle and reprioritise human connection.
Loneliness and a low sense of belonging remain major challenges with both young and the old leading them to use social media. E- safety developed in Australia restricts age related materials online such as high impact violence, self harm su***de and disordered eating, but they don't address social harms from AI chat bots.
It is proposed that age restricted material protections should extend to unintended social harm of conversation with social, analytically functional AIs.An authoritarian crackdown on chatbots may not be within the reach for a democratic nation. But, we can and should prioritise developing human connection skills in order to resist unhealthy over reliance on AI.

HPV VACCINATION : CONTROVERSY, DELIBERATIONS AND DELAY....
The Human Papilloma Virus ( HPV ) vaccine, incorporating all 13 antigens recommended by WHO for adolescent girls is a milestone and a decisive shift towards recognizing adolescence as a critical window for preventive health. It is also first vaccine offered exclusively to adolescents. This policy evolution acknowledges the fact that preventive measures cannot and should not conclude in early childhood or be restricted only to the female s*x as Oropharyngeal, A**l and Pe**le cancers seen in males also need HPV vaccination.
The first HPV vaccine was licensed in 2006, became available in India by early 2008 and got embroiled in controversy, deliberations and delay resulting in a prolonged phase of policy inertia and absence of public confidence, inspite of scientific evidence and repeated recommendations on its benefits by government and independent technical advisory bodies.
The cost of this delay, measured not merely in elapsed years witnessed many countries progressing with alacrity notably Australia and United Kingdom, which are now on the verge of eliminating cervical cancer as a public health problem.
In India, cervical cancer remains the second most common cancer in which persistent infection with high risk types 16 and 18 account for majority cases. Gardasil, the HPV vaccine for India has demonstrated 93 % to 100 % efficacy in a WHO endorsed dose for girls between 9 -14 years, aspires to achieve 90 % vaccination coverage as a part of its concerted effort to eliminate cervical cancer.
The reasons for delay in including HPV vaccination must be extensively studied by those bodies that were involved in their introduction namely the WHO, IMA ,FOGSI and other governmental agencies. The lessons learnt in the last 15 years also need to be understood and rectified. They are -
* Communication, which necessitates explanation on why the vaccine is being offered, how it functions and what safeguards are in place as scientific data alone cannot substitute trust.
* Ethical standards must be visibly demonstrable. Perceptions of exploitation can eclipse scientific merit.
* Prevention demands political consistency as success is measuredin cases where vaccination has failed to materialise. Enduring commitment determines lasting impact.
HPV vaccination, offered free alone will not suffice to eliminate cervical cancer. Screening through Pap smears or HPV testing must continue for unvaccinated older women too.
Simultaneously, India must broaden its preventive horizon and the delay in its introduction shouldvserve as a reminder that scientific advancement can be impede by mistrust Inadequate communication and political contestation.
If implemented with transparency, sustained political will and meaningful community involvement, the HPV vaccination programme can emerge as a public health achievement shielding women from preventable malignancies besides reaffirming the primacy of prevention in the National Health Policy.

THE JOY OF WRITING..
I confess not being comfortable at crowded places, am less social and happy in solitude where I do things I love to.
Writing became my passion, something that made me explore, express and an avenue to connect with people. Writing steered the intellectual growth in me igniting articulation and aptitude. It is common belief, that such people possess excellent communication skills because they form a face to face connect with people.
Writing is a chance to engage with people, speaking their mind and transform them into incredible communicators. People are busy nowadays having little time to listen and here writing helps to communicate.
At times, spoken words can be judgemental and deceptive. Here writing helps to overcome that particular flaw of oral communication. Written words spread the fragrance of love, catch the mood of enthusiasm and spike the vibes of auspiciousness.
Poetry or literature penned artistically and with sincerity comes as natural leaves to the tree, drop of honey or a ray of sunlight glancing from the clouds.
A writer is one who knows " what not to write ". The pleasure of being alone gives immense scope for creativity shaping his creation to culminate as a masterpiece.
A common tendency to overthink is a waste of imagination - create a meaningful poetry or literature as these don't make noise. When the pen screams on the paper, it is loud enough to sieze the attention of people forcing them to think and evolve bringing about reformative changes in the society.
Finally, writing is a silent screamer vehement enough to make far reaching impacts. A writer writes for pleasure and it not necessarily be read by anyone or everyone. He enjoys the thought process and its expression in exquisite words. Simply, writing is heavenly pleasure motivating the pen to write something he specializes in.
