KOTAK terbentuk 27 September 2004 dalam acara The Dreamband, KOTAK dipertemukan sejak audisi The Dreamband Jakarta. Nama KOTAK sendiri dipakai karna KOTAK berarti 4 sisi & 4 sudut yg bersatu menjadi bidang KOTAK yg menggambarkan 4 orang berbeda karakter namun menjadi 1 dalam musik
