基督教香港信義會外展專業服務隊 承蒙 香港賽馬會慈善信託基金捐助，推行本計劃，為期一年半，為元朗及屯門區共87間私營或自負盈虧的安老院舍提供服務。本隊透過樂齡設備及娛樂活動器材，改善院友的生活質素，更致力推動本地化的「人本自立支援」文化，以四要三不（多飲水、衡飲食、勤運動、能排便；不約束、不卧床、不用尿布）為原則，希望提升院友的自立、自主，培養院舍內「老有所為、老有所長」的氣氛，讓長者有尊嚴地渡過晚年。
