Piebaldism is a rare, inherited genetic condition characterized by patches of white skin (leukoderma) and white hair (poliosis) due to a congenital absence of pigment cells (melanocytes). It is an autosomal dominant disorder, most often caused by mutations in the KIT gene, affecting melanocyte development and migration. The most distinctive feature is a white forelock of hair, often in the frontal hairline, and irregular, symmetrical patches of absent pigment on the skin. Treatments are primarily cosmetic, and individuals with piebaldism should practice sun protection.

Piebaldism vs. Vitiligo

1. When it starts

Piebaldism β†’ Present from birth (congenital).

Vitiligo β†’ Usually appears later in life (childhood, adolescence, or adulthood).

2. Cause

Piebaldism β†’ Genetic condition (passed in families). The pigment cells (melanocytes) never develop in certain areas.

Vitiligo β†’ Autoimmune condition. The body attacks its own pigment cells, causing patches to lose color over time.

3. How it looks

Piebaldism β†’ Stable white patches that don’t spread. Often includes a white patch of hair (forelock) on the forehead and symmetrical patches on the body.

Vitiligo β†’ White patches that may spread gradually and appear anywhere on the body. Shape and size can change.

4. Family pattern

Piebaldism β†’ Often seen in multiple family members (inherited).

Vitiligo β†’ Can run in families, but not always directly inherited.

5. Progression

Piebaldism β†’ Does not progress. The patches stay the same for life.

Vitiligo β†’ Can progress slowly or quickly, sometimes spreading to large areas.
