Coming Home from Hospital? We’re Here to Help.

Being discharged from hospital can sometimes feel overwhelming, especially if you’re worried about how you’ll get home or what you’ll find when you arrive.

Our Hospital Transport Support Service provides FREE supported transport for patients aged 50 and over who are being discharged from Pinderfields, Pontefract Hospital or Dewsbury.

🚗 We can:
• Provide transport home from A&E or a hospital ward
• Provide a trained Age UK Wakefield District Support Worker to accompany you
• Es**rt you safely into your home
• Make sure you’re comfortable and settled
• Help with unexpected issues, such as faulty locks or heating problems
• Offer a hot drink or simple meal to help you settle in
• Make sure you’re offered further support if needed

The service operates 7 days a week and is completely FREE for eligible patients.

📋 How do I access the service?
Hospital nursing staff can make a referral for you and will check whether you’re eligible for the service.

If you or someone you know may benefit from this support, please speak to the hospital nursing staff before discharge.

Because getting home from hospital should be about more than just getting through the front door — it’s about making sure you’re safe, comfortable and supported when you get there.

A message from Trinity Medical Centre Wakefield about their move from PATCHS to a new system called SystmConnect on 5 October 2026. More information below 👇

📢 We’re changing our online consultation system.

We will be moving from PATCHS to a new system called SystmConnect from date 5th October 26.

SystmConnect will make it easier and quicker for you to send routine requests to the practice. It links straight to your GP record, is simpler to use and helps our team deal with requests more safely and efficiently.

PATCHS was given to all Wakefield practices to use, but after listening to feedback from patients, our PPG and staff, we’ve chosen a system that works better for our practice.

👉 When the change goes live, just use the same links on our website, they’ll take you to SystmConnect instead of PATCHS.

⚠️ Please remember: do not use SystmConnect for urgent, same day problems. Call the practice so we can help you straight away.
