“It’s incredibly powerful to see such a simple idea change behaviour” says Rhian Smith, and she really couldn’t be more right.

We all deserve peace and respect when the time comes. But in busy hospital wards full of noise and interruptions it can be difficult to achieve.

That is where Rhian and the Hospital Palliative Care Team at Cardiff and Vale University Health Board come in.

As finalists in our Service Innovator awards at our upcoming Professionals Excellence Awards, they are working tirelessly to improve things for people nearing the end of life – introducing Every Moment Matters, a visual symbol that discreetly signals when a patient is dying and needs a calm, dignified environment.

Developed with ward staff, non-clinical teams and families, the reaction to Every Moment Matters speaks for itself – with 100% of families feeling that the dandelion clock symbol helps create a more peaceful atmosphere for their loved ones.

Low-cost and gaining national interest for its ability to gently transform end-of-life care, it is an innovation that is all about helping staff to “provide the calm, respectful care patients and families deserve.”

“Mae gweld syniad syml yn newid ymddygiad yn rhywbeth mor rymus,” meddai Rhian Smith, ac ni allai fod yn fwy cywir.

Mae pob un ohonom yn haeddu heddwch a pharch pan ddaw’r amser. Ond, mewn wardiau ysbyty prysur sy’n llawn sŵn a tharfu, gall cyflawni hyn fod yn anodd.

Dyna rôl Rhian a Thîm Gofal Lliniarol yr Ysbyty ym Mwrdd Iechyd Prifysgol Caerdydd a’r Fro.

Maent wedi cyrraedd rownd derfynol ein Gwobrau Arloeswyr Gwasanaeth yn ein Gwobrau Rhagoriaeth Gweithwyr Proffesiynol sydd ar ddod ac maent yn gweithio’n ddiflino i wella pethau ar gyfer pobl sy’n nesáu at ddiwedd eu hoes – trwy gyflwyno Mae Pob Moment o Bwys, sef symbol gweledol sy’n rhoi arwydd gynnil i nodi pan fydd claf yn marw ac mae angen amgylchedd tawel, urddasol arno.

Wedi’i ddatblygu ar y cyd â staff y ward, timau nad ydynt yn rhai clinigol a theuluoedd, mae’r ymateb i gynllun Mae Pob Moment o Bwys yn siarad dros ei hun wrth i 100% o deuluoedd deimlo bod symbol cloc dant y llew yn helpu i greu awyrgylch mwy heddychlon ar gyfer eu hanwyliaid.

Gan fod yn gost isel ac ennyn diddordeb yn genedlaethol o ganlyniad i’w allu i drawsnewid gofal diwedd oes yn dawel, mae’n gynllun arloesol sy’n ymwneud â helpu aelodau staff i “ddarparu’r gofal tawel, parchus y mae cleifion a theuluoedd yn ei haeddu.”

With 60 people in Wales diagnosed with cancer every day - cancer isn’t waiting and neither should Wales. And yet it’s the only UK nation without a long-term cancer plan.

This is despite the fact we know countries with a plan improve outcomes and care, and that is why we are calling for a 10-year national cancer strategy in Wales.

The Senedd elections are happening on 7 May 2026 and as part of the build-up we chaired a political Hustings in partnership with the North Wales Prostate Cancer Support Group and the new Maggie’s centre in Clwyd, where the event took place.

Present were members of the Senedd and representation from many of the Welsh political parties, members of Maggie’s urological support group, Macmillan professionals and many others living with or affected by cancer.

Chairing on our behalf is External Affairs Manager Rhian Stangroom Teel; she said: “These hustings have been an opportunity to create a safe, inclusive space where people affected by cancer and their families can hear directly from Senedd candidates about their priorities, ask questions and feel their lived experience has been heard.”

Present was Neil, who received a workplace blood test with North Wales Police at 51, which showed high PSA levels. Neil went to his GP but had to insist himself that he was referred.

“That referral saved my life,” he said.

He was diagnosed with prostate cancer undergoing a Radical in Clwyd, but men now have to travel to England because his local health board lacks the necessary robotic equipment.

Because of this, along with the fact that there are few options for rehab and emotional peer support, Neil created three North Wales Prostate Cancer Support Groups across the region.

“It shouldn’t matter where in Wales you live, you should have access to the best treatment options and the right support – emotional and physical.”

For more information and insight into our call for a 10-year National Cancer Strategy in Wales, please read our Manifesto that we'll link in the comments.

Wrth i 60 o bobl yng Nghymru gael diagnosis o ganser bob dydd – nid yw canser yn aros ac ni ddylai Cymru aros chwaith. Ac eto, Cymru yw’r unig genedl yn y DU heb gynllun ganser hirdymor.

Mae hyn er gwaetha’r ffaith rydym yn gwybod bod gwledydd sydd â chynllun yn gwella canlyniadau a gofal a dyna pam rydym yn galw am strategaeth canser genedlaethol 10 mlynedd yng Nghymru.

Cynhelir etholiadau’r Senedd ar 7fed Mai 2026 ac, fel rhan o’r cyfnod sy’n arwain atynt, cadeiriwyd hustyngau gwleidyddol mewn partneriaeth â Grŵp Cymorth Canser y Prostad Gogledd Cymru a’r ganolfan Maggie’s newydd yn Sir Clwyd, lle y cynhaliwyd y digwyddiad.

Yn bresennol, roedd Aelodau o’r Senedd a chynrychiolaeth gan lawer o bleidiau gwleidyddol Cymru, aelodau o grŵp cymorth wrolegol Maggie’s, gweithwyr proffesiynol Macmillan a llawer o bobl eraill sy’n byw gyda chanser neu y mae canser wedi effeithio arnynt.

Yn gweithredu fel cadeirydd ar ein rhan yw Rheolwr Materion Allanol, Rhian Stangroom Teel, dywedodd: “Mae’r hustyngau hyn wedi bod yn gyfle i greu gofod diogel a chynhwysol lle y gall pobl y mae canser wedi effeithio arnynt a’u teuluoedd glywed yn uniongyrchol gan ymgeiswyr y Senedd ynghylch eu blaenoriaethau, gofyn cwestiynau a theimlo bod eu profiad bywyd wedi cael ei glywed.”

Yn bresennol roedd Neil, a gafodd brawf gwaed yn y gweithle gan Heddlu Gogledd Cymru yn 51 oed a ddangosodd lefelau uchel o PSA. Aeth Neil at ei Feddyg Teulu ond bu’n rhaid iddo fynnu ei fod yn cael ei atgyfeirio.

“Gwnaeth yr atgyfeiriad hwnnw achub fy mywyd,” dywedodd.

Cafodd ddiagnosis o ganser y prostad wrth gael prostadectomi radical yn Sir Clwyd, ond erbyn hyn y mae angen i lawer o ddynion deithio i Loegr am nad oes gan ei fwrdd iechyd lleol y cyfarpar roboteg angenrheidiol.

O ganlyniad i hyn, ynghyd â’r ffaith nid oes llawer o opsiynau ar gyfer adsefydlu a chymorth emosiynol gan gymheiriaid, sefydlodd Neil dri Grŵp Cymorth Canser y Prostad Gogledd Cymru ar draws y rhanbarth.

“Ni ddylai fod ots o ran ble yng Nghymru rydych chi’n byw, dylech chi gael mynediad at yr opsiynau triniaeth gorau a’r cymorth cywir – emosiynol a chorfforol.”

I gael mwy o wybodaeth a chipolwg ar ein galwad am Strategaeth Canser Genedlaethol 10 mlynedd yng Nghymru, darllenwch ein Maniffesto.

Cancer Community Meet-ups take place every other Wednesday at 10.30am, at Torello Lounge in Prestatyn. Expect a warm, friendly welcome and light refreshments - you’re welcome to bring a friend or family member along with you too.

These Meet-ups provide a relaxed and comfortable space for people with cancer and their loved ones to meet up with other people in their local community who may be going through similar experiences.

The idea behind Meet-ups is simple. Being around others who understand what you’re going through is an incredible source of support. Just knowing you’re not alone can provide reassurance and a sense of belonging. Unlike a more formal support group, Meet-ups have no agenda, no requirement to sign up, and no requirement to speak about cancer or even to speak at all.

10/11/2025

Rachel was diagnosed with bowel cancer in 2018 and underwent life-changing surgery to remove her uterus and have a stoma fitted. She quickly realised she wasn’t prepared, emotionally or in a practical sense, for how to manage. But on the hospital ward there was no one to turn to and the wrap around support she needed was missing.

“I didn’t feel emotionally strong enough for everything that was happening to me, and I needed for someone to see me as a person, not just a patient. I am seven years post-treatment but only now am I receiving psychological help via the NHS to process what I experienced. It shouldn’t be this way.”

More than one in four people with cancer in Wales (29%) either do not have the support of a specialist cancer nurse during their NHS care, or the support they receive from a specialist cancer nurse isn’t enough to meet their needs.

This month, we launched a manifesto to speak on behalf of the almost 190,000 people who are living with cancer in Wales and urge the Welsh government to put an end to the deepening crisis in Wales' cancer services.

The people of Wales deserve better. The system is stretched, the pressures are growing, and for people living with cancer, failures in their care are having lasting consequences. We must do better. And we can. Our manifesto was developed with people living with cancer and Macmillan Professionals. We have four asks to the next Welsh Government: to launch a new cancer strategy, overhaul support for the workforce, revolutionise NHS data use and – crucially – give us the person-centred support people deserve.

We’d like to thank everyone for joining us as we launched our manifesto for Wales. Macmillan is ready to be a partner in turning the tide and sparking a revolution for cancer care in Wales.

Cafodd Rachel ddiagnosis o ganser y coluddyn yn 2018. Cafodd lawdriniaeth a newidiodd ei bywyd i dynnu ei chroth a gosod stoma. Yn gyflym iawn, sylweddolodd nad oedd hi’n barod, yn emosiynol nac yn ymarferol, am sut i ymdopi. Ond, nid oedd unrhyw un ar ward yr ysbyty i droi ato ac roedd y cymorth cofleidiol roedd ei angen arni ar goll.

“Doeddwn i ddim yn teimlo’n ddigon cryf yn emosiynol am bopeth a oedd yn digwydd i mi ac roedd angen arnaf i rywun fy ngweld fel person, nid claf yn unig. Mae’n saith mlynedd ers i mi gael triniaeth ond dim ond nawr rwy’n cael cymorth seicolegol gan y GIG i brosesu’r hyn a brofais. Ni ddylai fod fel hyn.”

Mae dros un o bob pedwar person sydd â chanser yng Nghymru (29%) naill ai’n methu â chael cymorth nyrs canser arbenigol yn ystod eu gofal GIG neu nid yw’r cymorth y maent yn ei gael gan nyrs canser arbenigol yn ddigonol i fodloni eu hangenion.

Y mis hwn, lansiwyd maniffesto i siarad ar ran y bron 190,000 o bobl yng Nghymru sy’n byw gyda chanser ac i annog Llywodraeth Cymru i atal yr argyfwng sy’n dyfnhau yng ngwasanaethau canser Cymru.

Mae pobl Cymru yn haeddu gwell. Mae’r system dan bwysau cynyddol ac, i bobl sy’n byw gyda chanser, mae methiannau yn eu gofal yn cael canlyniadau hirdymor. Mae’n rhaid i ni wneud yn well. A gallwn ni wneud yn well. Cafodd ein maniffesto ei lunio gan bobl sy’n byw gyda chanser a Gweithwyr Proffesiynol Macmillan a gofynnwn am bedwar peth gan y Llywodraeth Cymru nesaf: lansio strategaeth canser newydd, gweddnewid cymorth ar gyfer y gweithlu, chwyldroi defnydd o ddata GIG ac – yn hanfodol – roi’r cymorth i ni sy’n canolbwyntio ar yr unigolyn rydym yn ei haeddu.

Hoffem ddiolch i bawb am ymuno â ni wrth i ni lansio ein maniffesto i Gymru. Mae Macmillan yn barod i fod yn bartner gan droi’r llanw a sbarduno chwyldro ar gyfer gofal canser yng Nghymru.
