Let’s Talk About Primary Care in High Green and Chapeltown.

Chapelgreen Practice are inviting our community to join them for a conversation about the future of primary care in High Green and Chapeltown.
They want to share the improvements they've made at Chapelgreen Practice, hear what matters most to you, answer your questions and explore how they can work together to meet the needs of our community.
This will be for current patients, potential new patients, local residents and community representatives.
