For babies born prematurely, leaving the hospital is only the beginning of their journey. โค๏ธ

In ๐Œ๐›๐š๐ฅ๐ž, ๐”๐ ๐š๐ง๐๐š, healthcare teams discovered that only ๐Ÿ’๐Ÿ“โ€“๐Ÿ“๐ŸŽ% of premature babies were returning for essential specialist follow-up appointments. Even more concerning, around ๐Ÿ๐ŸŽ% were dying in their communities after discharge. ๐Ÿ’”

The ๐๐š๐›๐ฒ ๐๐ฎ๐ฌ was developed to help address this challenge. ๐ŸšŒ By bringing follow-up care closer to families, it helps vulnerable newborns receive the attention they need without repeatedly travelling back to the referral hospital.

๐Ÿ‘ฉโš•๏ธ Through follow-up clinics and healthcare worker training, local medical teams are also becoming more confident in caring for these babies, helping reduce unnecessary referrals to larger hospitals.
