If you would like some help, advice and information on living well with dementia why not pop along to our dementia hub on the first Friday of the month, 10am-12noon at Leyland United Reformed Church.

Lots of information available for carers and family members and recently diagnosed.
Just turn up and we will aim to help. Next one is Friday 5th June 2026.

All volunteers are Dementia Friends, so you can bring your loved one if you need to.

Alongside the Hub is a social group called NEXT STEPS. Age UK Lancashire are there for you to meet others in a similar position and benefit from shared experiences, mutual support and social connections.

Dementia awareness sessions in schools !

We are working with all South Ribble Primary Schools to help their pupils have a better awareness and understanding of dementia.

The Dementia Community Connection (DCC) have put together with the help of others a package that delivers a basic understanding to primary school aged children of dementia.

Our volunteer Tim will be available to come to the school and speak to pupils and raise their understanding.

Are you aware of the forms and tools available for people living with dementia which provides important information about that person?

They are several forms, which include:
Forget-Me-Not from for dementia – this is a vital tool for patients with dementia, providing healthcare staff with an insight into the patient’s preferences and needs.
This form is completed by the patient or their carer and includes information that should be known about the person and how to plan their care.
This form helps ensure that the patient receives the best possible care tailored to their individual needs.

Hospital passport – a hospital passport is a personalised document that provides healthcare staff with essential information about a patients needs, preferences and communication style to ensure safe and effective care.
A hospital passport is designed to help hospital staff understand a patient quickly, and would be a great benefit for someone living with dementia.
It ensures that healthcare professionals can provide care in a way that reflects their needs and makes hospital visits less stressful and more efficient. It can support reasonable adjustments rights.
Patients or carers complete the hospital passport, which is then carried to hospital appointments or stays.
To obtain a hospital passport speak to your GP. Also visit NHS England website.

Hidden disability lanyards – this is a discreet tool that help individuals communicate their needs without revealing their specific conditions.
There are various types – including the hidden disabilities sunflower.
The use of these lanyards is growing.

Message in a bottle – this is beneficial for people living with dementia. It provides a simple and effective way for individuals to keep their essential personal and medical details readily accessible in an emergency.
A form is completed and popped in a plastic bottle and put in the fridge.
The stickers on the fridge and door alert emergency responders to the presence of the bottle

Tomorrow – we are bringing House of Memories on the Road to South Ribble.
Come along on Tuesday 19th May between 11am and 4pm and experience a mobile museum which is designed to create memorable experiences for vulnerable and socially isolated people and people living with dementia.

Everyone welcome. The mobile museum will be parked on the car park of Bamber Bridge Leisure Centre, PR5 6YJ.

Next week we are holding an event which we would love people living with dementia and their loved ones, and socially isolated people to come along to.

The mobile museum will be parked on Bamber Bridge Leisure Centre car park on Tuesday 19th May, 11am-4pm.

Everyone is welcome – it is a free event. Come along and see the use of digital technology of more than 30 immersive experiences for sight, sounds and smells of the past.

Come along to our mobile museum event on Tuesday 19th May and experience some memories of the past.

House of Memories on the Road is coming to Bamber Bridge Leisure Centre on the 19th between 11am and 4pm and you can enjoy some experiences of sights, smells of the past.

If you are caring for a loved one with dementia, or would just like to experience it for yourself, you are all welcome - wrap yourselves up in warm clothing and come along.

House of Memories On The Road is National Museums Liverpool's touring 30 metre square mobile museum that opens up into an immersive cinema and activity space.

We are bringing the museum to South Ribble…

It has been designed to create memorable experiences for vulnerable and socially isolated people and people living with dementia. Why not bring along your loved one and let them enjoy a magical experience in a safe and intimate way, exploring memories of the past and bringing joy to the present.

Using digital technology, House of Memories On The Road has more than 30 immersive experiences for sights, sounds, smells of the past through a virtual ‘front door’.

Come along to Bamber Bridge Leisure Centre car park (Withy Grove park) PR5 6YJ on Tuesday 19th May between 11am and 4pm. Everyone is welcome.

 Are you concerned about a family member or friend and would like help of where to go and who can help?
 Have you had a recent diagnosis and would like to be pointed in the right direction where to go next?

If the answer to any of these is yes, then pop along to the dementia hub at Leyland United Reformed Church on Hough Lane and we have the information to provide to you to help and take away and make contact with the relevant organisation you need.

We will be there on the first Friday of the month. 10am-12noon.

Our volunteers are Dementia Friends. And now alongside the Hub is a social group called NEXT STEPS. Age UK Lancashire are there for you to meet others in a similar position and benefit from shared experiences, mutual support and social connections.

Are you interested in cycling and getting out in the fresh air?

There are a number of cycling activities available for the community to book onto.
These include small group training and guided rides as well as cycle servicing and maintenance help

We are a national award winning Bikeability Training Provider working with Lancashire County Council to deliver cycling training from children aged 4 all the up to young people aged 16.
