The Rosemary Foundation - Hospice at Home The Rosemary Foundation provides palliative care at home to those with a life limiting illness.

How the Rosemary Foundation works:

Our service is provided by a team of qualified and experienced nurses, counsellors and healthcare assistants, supported by providers of clerical, financial and administrative services as well as a Board of Trustees and a Management Team, who have responsibility for day to day operations. Patients who are referred to us are assessed by one of our nurses, who will

visit the patient and their family, and also contact other healthcare professionals involved, particularly the GP and District Nurse, to assess the type and degree of care required and how this may best be provided. A care plan is drawn up with the patient and care arranged, with one named carer appointed to take overall responsibility in each case. Anyone may refer a patient to us, but normally it is not possible to take on the care of any patient without the agreement of their GP.

Our Story

How the Rosemary Foundation works: Our service is provided by a team of qualified and experienced specialist nurses, counsellors and care assistants, supported by providers of clerical, financial and administrative services, many of whom are volunteers, working together from our offices at 35 Lavant Street in Petersfield. The Board of Trustees and the Management Committee (with responsibility for day to day operations) also meet regularly.

We work within a 15-mile radius of Petersfield, without charge to our patients or their families, in close cooperation with other palliative and end of life care professionals locally such as the community nursing teams, Rowans Hospice and the flagship Macmillan service in Midhurst. We operate on a 24/7 basis, 365 days a year, and the out-of-hours, on-call service is particularly valued by our patients, their families and the other organisations wesupport. Patients who are referred to us are assessed by one of our nurses, who will visit the patient and their family, and also contact other healthcare professionals involved, particularly the GP and District Nurse, to assess the type and degree of care required and how this may best be provided. A care plan is drawn up with the patient and care arranged, with one named carer appointed to take overall responsibility in each case. Anyone may refer a patient to us, but normally it is not possible to take on the care of any patient without the agreement of their GP.

In addition, we provide a Volunteer Visiting service to our patients, bereavement support and counselling to patients families together with ongoing carer support as and when required. We also work with other local organisations, such as Artscape, to provide other activities and support to those who have used our services.

We are entirely voluntarily funded.
