Wellington Support provides Nurse and Occupational Therapist lead community support packages for individuals who are suffering with mental health difficulties. We specialise in supporting individuals who have histories of serious risk to themselves or others; and still require medium to high levels of ongoing community support, structure, supervision and risk management. We provide support with al

l the activities of daily living as well as support with medication, benefits and support in finding suitable education, leisure and work opportunities. Wellington Support can also offer a corporate appointee for individuals who need structure and support to manage their personal finances. We work in partnership with all relevant health/social care professionals to support an individual’s community re-enablement process. By providing both supported housing and independent flats, Wellington Support can provide a choice of accommodation and support options for people leaving institutional care who present with different levels of needs; and who require differing levels of support.

Community Support Coordinator - Northampton

Wellington Support provides individual packages of care in supported accommodation to people experiencing mental health difficulties. We have various types of accommodation and work towards enabling people to reach individual goals, integrate back into the community while maintaining independence and dignity.
The role will include supporting people to:
• Build or develop living skills needed for independent living such as budgeting, cooking, cleaning.
• Maintain personal schedules and attend appointments
• Manage medication
• Research and attend vocational and educational placements.
• Cope with crisis and decline in health.
• Build strategic relapse prevention plans and plan for future personal development.
Wellington Support are looking for applicants with experience of mental health or similar for this role and a pragmatic mature approach. Applicants must be able to display excellent written and verbal communication skills. Proven confidence and organisational skills needed to complete lone working under minimal supervision whilst developing therapeutic relationships with people displaying challenging behaviours.
Applicants must hold or be willing to complete a QCF/NVQ level 2/3 and undertake an enhanced DBS disclosure.
