Dementia Support in the CommunityThe Dementia Support Trust North and South Molton is a registered charity ( 1197816)We work collaboratively with likeminded individuals, organisations and community groups to offer carefully tailored, compassionate, inclusive and therapeutic sup

port to ensure that everyone affected by dementia has support to continue to lead lives which are creative, active, productive and meaningful.About usIn 2021 Harriet set up the Dementia Support Trust as there was a gap in free dementia support and activities in the local community which was accessible for all. Harriet has 15 years experience in working with those affected by dementia as a Dementia Care Home Manager and trainer and is passionate about providing a tailored, meaningful and therapeutic approach to activities which gives free respite to loved ones and carers. The Dementia Support Trust provides free activity sessions to people affected by dementia. Together a detailed care plan is created to enable people to reconnect with past hobbies and interests and we listen to who and what is important to them. Members have enjoyed music sessions, flower arranging, gardening, baking, crafts, knitting, reminiscing, painting, woodwork and visiting local attractions, coffee shops and garden centres. Our aim is to ensure that all our sessions are inclusive and accessible, and we offer these sessions at a location of the members choice.Home Visits Those with advancing dementia or low mobility may prefer a home visit and those that have perhaps felt isolated during Covid and would like to improve their confidence and meet new people, may like to attend our group sessions.Group Sessions Our groups are welcoming, fun and have a maximum of 6 people in each with similar hobbies and interests to create a person centred friendly environment where people can feel comfortable, confident and to socialise and be themselves. To book a home visit or join one of our group sessions , call Harriet 07504660177We fully support our members in reaching their goals and collaborate closely with the local community and organisations to assist them further, such as GP’s, social services, care providers and carer and dementia charities. We have regular visits from local clubs, the local library, museum, and crafters that would like to share their skills to the groups. Involving the whole community is important to the charity. We have our own liability insurance and are DBS checked.

The Dementia Outreach Team gives advice and support to carers, families and professionals within the community of North Devon for people living with Dementia.

This is provided in nursing and residential homes and individuals’ own homes. We provide our experience and skills to carers and families, who are experiencing difficulties in helping people living with dementia carry out their activities of daily living that they can find challenging.

We aim to provide timely, individualised person-centred interventions to meet often complex needs and behaviours that other people may find difficult. This reduces risk, improves quality of life, and relieves carer stress and most importantly maintains positive relationships.
