National Physical Health and Long-Term Mental Health Conditions Survey

📢 Your experience can help shape future support across Wales.

NHS Wales is inviting people living in Wales who have experience of a long-term mental health condition to take part in a national survey about physical health.

People living with long-term mental health conditions can face additional challenges in maintaining good physical health. This survey aims to better understand what helps, what gets in the way, and how health and support services could work better together.

Your experiences will help inform a National Improvement Plan for Wales, with the aim of improving physical health, reducing health inequalities and helping people live longer, healthier lives.

💙 Who can take part?
Anyone living in Wales with experience of a long-term mental health condition, whether or not you currently use mental health services.

Taking part is voluntary and confidential, with responses reported only in summary form.

Your voice and your experience can help make a difference.
