We are committed to raising peoples awareness to the challenges that face people when dealing with death, dying and loss, and to promote a compassionate response that will support people on their final journey and the continued journey for those they leave behind. These people we understand to be vulnerable and valued. We also know that talking and planning for death and dying helps us to live bet

ter. That is why we promote the importance of seeing end of life as part of life and making decisions about our own end of life journey. We believe that death and dying are social issues as well as a medical issue.

This simple booklet is a lovely tool where you can plan for tomorrow.

You can record your personal wishes for end of life care and funeral plans; giving you peace of mind.

You donโ€™t have to fill it in all at once; and you can come back to change or update it at any time.

Once completed, you should keep it in a safe place.

Most importantly, let a loved one or trusted friend know where it is stored so they can access it easily when the time comes.

As part of Dying Matters Week 2026, Foyle Hospice launched 'Wee Chat' and we are delighted to have our 'Plan Ahead' resources featured.

How does Time for a Wee Chat work?

It is simple, private and designed to feel gentle.

You type your first name.

You choose what kind of conversation you are thinking about.

You answer five short questions.

You receive a conversation starter to help you begin the conversation.

There are no long forms.

No complicated language.

No pressure to do anything before you feel ready.

Your name is not stored.

The tool does not ask for contact details.

It is there to help you gather your thoughts and find a few words.

It may only take a few minutes, but it could help begin a conversation that really matters.
