Join our Social Prescribing team for a six-month project that will connect people living with musculoskeletal conditions and/or mental health needs with practical, personalised support to move towards their goals.

You will build trusted relationships, help people strengthen their confidence and wellbeing, and open up opportunities through the right services, community networks and employment support.

This is an opportunity to be part of a new project from the beginning: testing what works, building partnerships across Merton and South London, and helping create learning that could shape future support for local communities.
