One of the main aims of our Health Pooled Fund (HPF) Programme is to support community-based health services in fragile and conflict-affected settings in South Sudan with a focus on reducing maternal and child mortality rates.

The programme does this through the continued delivery of a basic package of health and nutrition services, promoting community engagement in health as a public good.

Creating health systems that reach every community is critical to achieving universal health coverage and the delivery of the Sustainable Development Goals (SDGs), particularly SDG 3 โ€“ good health and wellbeing.

In 2017, the Ministry of Health (MoH) in South Sudan launched the Boma Health Initiative (BHI), a national scale Community Health programme that aims to strengthen the health system in South Sudan.

The initiative has demonstrated enormous potential to improve health and community systems in South Sudan, including a reduction in mortality and morbidity of children under five years and maternal mortality rates, improved immunisation coverage and increased prevention of the diseases such as malaria and pneumonia.

Since 2012, the Health Pooled Fund South Sudan delivers improved health and wellbeing for the population of , specifically a reduction in maternal deaths and deaths of children under five.

To achieve this, the programme supported health facilities across eight States of South Sudan and over 4,400 Boma Health Workers delivering health services in over 746 Bomas in 55 counties (including Unity State). Since 2022, the programme works in seven States.

In the period of implementation, the programme procured, stored and distributed 22 consignments of life-saving drugs worth 2 million each and set up a procurement and logistics system that consistently delivered three consignments of medicines each year, despite 80% of the road network being inaccessible during the rainy season.

The HPF Consortium is led by Crown Agents, supported by Montrose International, International Procurement Agency b.v. (IPA) and nine implementing partners. It is funded by six donors, led by the Foreign, Commonwealth & Development Office. Other donors include USAID - US Agency for International Development, the European Union, Canada, Gavi, the Vaccine Alliance and the Embassy of Sweden Office in South Sudan.

Each year, routine immunisations prevent 3.5 to 5 million global deaths from vaccine preventable diseases (VPD) such as diphtheria, tetanus, pertussis, influenza and measles.

are the most effective way to prevent many infectious diseases. Without vaccines, they spread quickly.

In , over 10,000 cases of cholera, 5,000 of measles, 8,000 of dengue, and a staggering 1.2 million cases of malaria were reported in 2024.

To tackle this, the Health Pooled Fund Programme, led by Crown Agents in collaboration with Montrose International and IPA (International Procurement Agency b.v.) and funded by the Foreign, Commonwealth & Development Office, USAID - US Agency for International Development, the European Union, the Government of Canada, Embassy of Sweden Office in South Sudan and Gavi, the Vaccine Alliance, has worked since 2017 towards increasing routine immunisation across seven states in collaboration with the South Sudanese Government. Its specific focus is to expand immunisation services to reach zero-dose, under-immunised children.

To reach remote communities across the country, the team regularly battles flooding, an inaccessible road network and internal armed conflict. Despite these challenges, the programme has vaccinated over 2,400,000 children under one year with the third pentavalent/DPT vaccine, and increased coverage by over 23% across the 7 states the programme is active in.

In Haridwar City, located in Uttarakhand State, India, the disposal of Municipal Solid Waste (MSW) poses significant challenges for the municipality, specifically in the realm of solid waste management. This gives rise to broader environmental and health risks for all residents, such as a spread of infections and diseases through attracting rodents and other creatures and pollution from chemicals released through landfills.

To remedy this, Crown Agents and the Centre for Environment Education Society (CEE), with the support of Roche pharma Pvt. Ltd India, have joined forces with the municipality, the Nagar Nigam (NN), to implement project SWACHH. The objective of project SWACHH is to facilitate the separation of organic (wet) waste from dry waste for subsequent collection and conversion into compost.

In year one, the project focusses on efficient management of organic waste across the Bulk Waste Generators (BWGs) like ashrams, hotels and dhabha (food markets). So far, the project has installed 240 bio-composters across the city and is leading a widespread information, education, and communication campaign.

๐Ÿ“ข Exciting update! The FCDO has completed the Year 4 annual review of our HPF programme, and we're thrilled to share that our overall performance has been rated 'A'! ๐ŸŒŸ

We're now busy implementing the recommendations from the review, and we can't wait to see the positive impact they'll have. ๐Ÿš‘

In addition, we recently hosted two engaging learning round table discussions with our donors and implementing partners. We dived into important topics like supply chain management, data for decision making, maternal health, and capacity building initiatives. ๐ŸŒ

But the learning doesn't stop there! We're continuing these round table discussions to cover even more crucial areas such as the boma health initiative, child health, family planning, gender equality, and social inclusion. Together, we're working towards a brighter future for all. ๐Ÿค

Stay tuned for more updates on our progress ๐ŸŽ‰

Our Health Pooled Fund South Sudan delivers essential life-saving drugs across 80% of South Sudan. The programme is working in one of the most challenging logistical environments in the world, facing armed conflict, flooding, and an incomplete road network of which 60% is not accessible during the rainy season.

Yet, 83% of South Sudan's population lives in rural areas of the country and are in urgent need of essential medication.

To ensure they have access to life-saving vaccines, HPF provides support to South Sudanโ€™s national immunisation campaign by promoting routine immunisation across 7 out of 10 states. To support the initiative, Gavi, the Vaccine Alliance, received a donation from the Toyota Motor Corporation for a vaccine transport vehicle, fitted with a vaccine fridge which ensures efficient and effective delivery of safe vaccines.

Go on the journey with our teams in South Sudan to learn about this and other challenges of operating across the country: https://bit.ly/44ejKWo

HPF is funded by the Foreign, Commonwealth & Development Office, Gavi, the Vaccine Alliance, USAID - US Agency for International Development, Embassy of Sweden Office in South Sudan, the Government of Canada, the European Union, and supported by Montrose International and 9 implementing partners.

Imagine a context in which the next health facility is a day away by foot. Imagine that even if you embark on this journey, carrying your baby, your travel may be affected by floods, or you may run into armed groups. Image after hours of walking, you arrive at the clinic, and it has been flooded.

This is the reality for many women in South Sudan. Around 83% of the population in South Sudan lives in rural areas, most of them in scattered hamlets and cattle camps.

Since 2017, the Health Pooled Fund Programme, led by Crown Agents in collaboration with Montrose International and the International Procurement Agency b.v. and funded by the Foreign, Commonwealth & Development Office, USAID - US Agency for International Development, the European Union, Canada, the Embassy of Sweden Office in South Sudan and Gavi, the Vaccine Alliance, has worked with healthcare staff at clinics, as well as community chiefs and the Boma Health Workers to increase immunisation rates across the country.
