Dear Parent
We hope this email finds you and yours well . We would be really grateful if you could take some time to complete 2 surveys from the list below .
You will see there are a number of surveys for you to select .
We apologise in advance for the length of the surveys . As you will all be aware our life's as parents to a child/young person involves many services we are often needing to communicate with or about .
Please choose the survey that relates to your child's age . There is also a survey relating to home education which covers all ages . Lastly there is a survey about us, Brent Parent Carer Forum , we would like to receive feedback on our support to you .
What will we do with your feedback ?
We will collate the feedback and produce a report that we will take the Brents Inclusion Board and to all the relevant service leads . We will ask for a response to your feedback and where their themes in needs etc we will ask for an improvement plan . We will feed back to all our members .
