Really proud of the impact our Caring Connections Reablement Team continues to make supporting people experiencing homelessness across Liverpool.
Over the period we have reviewed, our team has provided:
20,740 hours of one-to-one reablement support
240 people experiencing homelessness supported
Behind those figures is some incredibly important work.
Homelessness rarely exists in isolation. People can also be experiencing poor health, addiction, complex care needs and a loss of independence. Our approach is to provide consistent one-to-one support, building trust and working alongside people to help them regain confidence, develop everyday living skills and become more independent.
That can mean supporting someone with personal care, preparing meals or maintaining their living environment. It can mean helping someone engage with other services or take those first steps towards recovery. In some circumstances, our teams are supporting people with extremely complex and deteriorating health needs.
And we are seeing some fantastic outcomes.
People moving into their own homes.
People entering rehabilitation.
People overcoming alcohol dependency.
People reconnecting with their children and families.
People regaining their independence.

20,740 hours of one-to-one support means 20,740 hours spent alongside people who need someone in their corner.
