Due to continued growth and increasing demand, we’re looking for additional Support Brokers to join our expanding team.

This is more than just a support role. As a Support Broker, you’ll work alongside people leaving hospital to help them regain independence, improve wellbeing and access the support, services and community connections they need to live well at home.

Are you passionate about helping people live independently and supporting them to regain confidence after leaving hospital?

We are looking for a compassionate and motivated Personalised Support Specialist to join our growing team, providing person-centred support to people across North & East Cornwall.

πŸ’· Β£26,000 per annum
πŸš— Mileage paid at 55p per mile
πŸ“ Community-based across North & East Cornwall
⏰ 37 hours per week

Due to the travelling requirements of this community-based role, candidates would ideally be located in either Callington, Liskeard, Saltash, Launceston or within easy travelling distance of these areas.

πŸ’™ This rewarding role involves:

β€’ Supporting people to recover safely at home following hospital discharge
β€’ Carrying out home visits and holistic assessments
β€’ Helping people access housing, benefits, adaptations and community support
β€’ Providing short-term practical support, such as shopping or meal preparation, until longer-term support is in place
β€’ Empowering people to maintain their independence and improve their wellbeing

Please note: This role does not provide personal care.

disAbility Cornwall & IoS has been invited to give evidence to the Timms Review on Personal Independence Payment (PIP) as part of its national engagement with Disabled People’s Organisations.

We want to make sure the views we take into that discussion are informed by Disabled people, people with long-term health conditions, carers and others with direct experience of PIP across Cornwall and the Isles of Scilly.

So, we need to hear from you.

If you could redesign PIP, what would it look like – and what should it achieve?

We’d particularly like to hear your thoughts on:

πŸ‘‰ What needs to change about the application and assessment process?
πŸ‘‰ How could assessments better recognise fluctuating, invisible and long-term conditions?
πŸ‘‰ What would make the system fairer, more accessible and less stressful?
πŸ‘‰ What should PIP actually enable Disabled people to do?
πŸ‘‰ If you could change one thing about PIP, what would it be?

Your experiences, ideas and suggestions will help inform the evidence we give directly to members of the Review’s steering group.

The Review will use evidence from these sessions to help shape its recommendations ahead of its final report in the autumn.

The Power of Compassionate Support 🌟

Following the loss of our client's spouse, we provided them with both practical and emotional support.

Prior to the funeral, the client disclosed they had nobody to accompany them and was upset that they had been unable to arrange a haircut. We found a hairdresser the same day who was able to support our client in preparation for the funeral.

On the day of the funeral, staff accompanied the client, supported them throughout the service, and later took them home for tea, cake and an opportunity to talk about their spouse and reminisce.

Longer-term support was then arranged to promote stability and independence, including Meals on Wheels, reinstatement of weekly PA domestic support, a dementia clock, and a long-term four-times-daily package of care.

This case demonstrates the value of person-centred, holistic support. By responding to both practical and emotional needs, coordinated intervention helped the client maintain their dignity, wellbeing, and independence during a time of significant change and loss. πŸ’™
