Cleobury Compassionate Communities

Cleobury Compassionate Communities We are a voluntary service set up to offer help & support for vulnerable and frail members of the Cleobury Mortimer community.

Cleobury Community Services (Co Co) recruits volunteers who offer a couple of hours of their time each week & partners them with clients referred by the local medical practice. Help is given according to the need of the client & the ability of the volunteer, but may include help with gardening or shopping, taking to an appointment or simply providing company.

Join me for a friendly, informal meet-up where anyone is welcome to pull up a chair, share a chat and enjoy some company! 🫶

It’s a simple community campaign to tackle loneliness so please do spread the word!

THIS DUTCH NURSING HOME GLUED RESIDENTS' OLD FRONT DOORS TO THEIR ROOM DOORS 🚪

A Dutch company called True Doors places life-size stickers of each resident's real front door on the entrance to their nursing home room.

It began in 2014 at a South Holland facility where most residents have dementia.

Instead of identical clinical doors, each person now sees the color and details of the door they lived behind for decades.

The decals cut disorientation and help residents find their room independently.

Caregivers noticed something else: residents started talking more, sharing old memories with staff and each other.

The Dutch Ministry of Health featured it as a model worth spreading.

If someone you love is in memory care, would a door like this have made a difference?

Our practice is proud to have a range of Additional Roles Reimbursement Scheme (ARRS) staff supporting patient care. These team members bring specialist skills that help us offer the right care, at the right time, from the right professional.

From clinical pharmacists and social prescribers to physiotherapists, care coordinators and more, our wider team works together to improve access, reduce waiting times, and support your health and wellbeing in different ways.

Take a look at the poster below which introduces each role, and how they can help you. You may be offered an appointment with one of these professionals as part of your care, by making the most of our full team, we can ensure you receive the best possible support.

This evening I was honoured to represent Cleobury Compassionate Communities at the Carol Service hosted by the Lord Lieutenant of Shropshire at St Mary's Church, Cleobury Mortimer. It was wonderful to see the work of volunteers recognised and that includes all of our amazing volunteers, although I won't be sharing the chocolate 🤣
