Every outcome in the epidermis begins with the Keratinocyte.

From hydration and barrier function to pigmentation, sensitivity, ageing and repair, the health of the epidermis is ultimately determined by the lifecycle of a single cell, the keratinocyte.

The journey begins in the stratum basale, where keratinocytes are born through mitosis. These newly formed cells gradually migrate upwards through the epidermis, undergoing a carefully orchestrated process of differentiation. As keratinocytes move through the stratum spinosum and stratum granulosum, they begin producing keratin, natural moisturising factors and the lipid components required for barrier formation. They also communicate with neighbouring cells, helping coordinate immune responses, pigmentation and epidermal repair.

By the time they reach the stratum corneum, these cells have transformed into corneocytes, flattened, highly specialised structures that form the skin's primary protective barrier. Together with the lipid matrix, they create the "brick and mortar" architecture responsible for reducing water loss, defending against environmental stressors and maintaining epidermal integrity.

When keratinocyte maturation is disrupted, the consequences are far reaching. Barrier dysfunction, increased TEWL, impaired desquamation, sensitivity, inflammation, pigmentation irregularities and delayed recovery can all follow.

This is why Corneotherapy places such importance on supporting normal epidermal function rather than simply treating symptoms. By understanding and respecting the keratinocyte lifecycle, we support the very foundation of healthy skin.

Barrier repair is not a trend. It’s a biological requirement.

The skin doesn't wake up one day and decide it needs repairing. When we see inflammation, sensitivity, dehydration, impaired healing, redness or chronic skin dysfunction, these are often signs that the skin's natural defence systems are no longer functioning optimally.
Yet in a world obsessed with stronger actives, more aggressive treatments and faster results, barrier repair is sometimes dismissed as a trend.

The reality is quite the opposite. The skin barrier is fundamental to skin health. Without an intact acid mantle, cohesive corneocytes and well organised epidermal lipids, the skin cannot effectively regulate water loss, defend itself against environmental stressors or maintain normal cellular function.

This is why barrier repair sits at the heart of Corneotherapy. Not because it's fashionable. Not because it's popular. But because healthy skin function depends on it. When we support the barrier, we support the skin's ability to do what it was biologically designed to do.

Is it really acne… or is it barrier dysfunction?

One of the biggest mistakes we see in skin treatment is assuming every breakout needs to be attacked aggressively.
In Corneotherapy, we understand that many acne presentations are deeply connected to barrier dysfunction. In fact, sometimes what looks like “stubborn acne” is actually an impaired barrier driving inflammation, sensitivity and delayed healing.

A true acne condition involves factors like excess sebum production, follicular congestion, bacterial imbalance and inflammation. But when the barrier becomes compromised, the skin becomes more reactive, more inflamed and less capable of regulating itself effectively.
This is where the overlap happens. Over cleansing, excessive exfoliation, harsh actives and aggressive treatment approaches can disrupt the stratum corneum and increase TEWL, leaving the skin in a constant cycle of inflammation and reactivity. The result? Breakouts that struggle to heal and skin that appears increasingly sensitised over time.

Corneotherapy is built on the principle of repairing and maintaining the skin barrier defence systems — not stripping, inflaming or over stimulating the skin.

2️⃣ The stratum corneum is biologically active.
It’s not just “dead skin.” The outermost layer of the epidermis communicates with deeper layers and plays a critical role in inflammation, immunity and regeneration.

3️⃣ Barrier damage changes everything.
When corneocytes, lipids and the acid mantle are compromised, TEWL increases, inflammation escalates and skin conditions become harder to manage.

Let’s Talk About Lipids

When it comes to skin barrier health, it’s not just about adding lipids, it’s about the right balance of lipids. In corneotherapy, the skin’s barrier function depends heavily on three essential epidermal lipids... ceramides, cholesterol and free fatty acids. Together, they form the lamellar structure that keeps the skin resilient, hydrated and protected.

One of the most discussed concepts in barrier repair is the lipid molar ratio, commonly referenced as 3 : 1 : 1

Why does this matter? Because the skin recognises balance. When lipids are delivered in proportions that closely mimic physiological skin function, the barrier is better able to recover, reduce transepidermal water loss (TEWL), maintain hydration, and support overall skin resilience.

Applying lipids in incorrect ratios may not provide the same reparative benefit and, in some cases, may even compromise barrier recovery. Donna Glazer is a Corneotherapist, Skin Science Educator and Founder & CEO of I.D. Skincare, United Kingdom. An award winning clinician and respected corneotherapy educator, Donna has spent nearly 40 years in the beauty industry, with the last three decades dedicated exclusively to skin.

Early in her career, Donna had the privilege of learning from Florence Barrett-Hill, inaugural I.A.C. Chair, giving her exposure to corneotherapy principles long before she even knew the term. She later transformed her beauty business into a dedicated skin clinic that continues to operate today, while remaining a hands on skincare professional throughout her career.

For the past 17 years, Donna has also distributed a leading corneotherapeutic product line, deepening her understanding of how the skin responds when products are truly aligned with corneotherapeutic values. Seeing corneotherapy in action on real skin, in real time, has shaped her work and led her into skin education, supporting both professionals and clients alike.
