𝗤𝘂𝗶𝘇 𝘁𝗶𝗺𝗲!

An 83-year-old woman with ulcerative colitis presents with a 1-year history of a progressively enlarging lesion on the left lower extremity.

Initially, the lesion appeared as a poorly demarcated, non-tender erythematous plaque. Multiple courses of antibiotics were prescribed for suspected cellulitis, but no improvement was observed.

💊 Immune checkpoint inhibitors (ICIs) have transformed melanoma care, but cutaneous adverse events can be challenging to manage.

In their case report, Christou et al. describe a patient who developed a pityriasis lichenoides (PL)-like eruption induced by pembrolizumab.

The patient was successfully treated with dupilumab, highlighting its potential as a targeted option for managing cutaneous immune-related adverse events, with minimal impact on immune checkpoint inhibitor efficacy.

In this contribution, Traini et al. describe the case of a 58-year-old patient with erythematous atrophic plaques who underwent LC-OCT imaging, revealing skin abnormalities suggestive of mycosis fungoides.

A targeted biopsy confirmed the diagnosis at the plaque stage. The report highlights the potential of LC-OCT to improve diagnostic accuracy and optimize biopsy site selection in challenging dermatological cases.

A 70-year-old man with a history of extensive travel to South and Southeast Asia presents with diffuse erythematous plaques, nodules, and hyperkeratotic lesions on the trunk, limbs, face, and earlobes, alongside progressive peripheral neuropathy and unexplained weight loss.

🤔 What's the diagnosis?

This case report by Dixit et al. describes a 53-year-old woman with necrobiotic xanthogranuloma (NXG) associated with IgG lambda monoclonal gammopathy of undetermined significance (MGUS), presenting with progressive cutaneous and ocular involvement. Given the lack of standardized therapies for NXG, pegylated interferon alpha-2a (PEG-IFN-α-2a) was initiated as first-line treatment.

The patient showed significant clinical improvement and reduction of paraproteinemia at a low weekly dose, with manageable side effects. The case suggests that PEG-IFN-α-2a may be a promising and well-tolerated therapeutic option for progressive NXG in patients with MGUS.

Could dupilumab treat pemphigus? A case report by Mateos Rico et al. shows a patient with severe pemphigus vulgaris (PV) achieving complete remission after 6 months on dupilumab.
