In this contribution, Traini et al. describe the case of a 58-year-old patient with erythematous atrophic plaques who underwent LC-OCT imaging, revealing skin abnormalities suggestive of mycosis fungoides.

A targeted biopsy confirmed the diagnosis at the plaque stage. The report highlights the potential of LC-OCT to improve diagnostic accuracy and optimize biopsy site selection in challenging dermatological cases.

This case report by Dixit et al. describes a 53-year-old woman with necrobiotic xanthogranuloma (NXG) associated with IgG lambda monoclonal gammopathy of undetermined significance (MGUS), presenting with progressive cutaneous and ocular involvement. Given the lack of standardized therapies for NXG, pegylated interferon alpha-2a (PEG-IFN-α-2a) was initiated as first-line treatment.

The patient showed significant clinical improvement and reduction of paraproteinemia at a low weekly dose, with manageable side effects. The case suggests that PEG-IFN-α-2a may be a promising and well-tolerated therapeutic option for progressive NXG in patients with MGUS.

A 13-year-old male presented with diffuse, pruritic plaques and patches of alopecia with fine scaling on the scalp. Discrete pustules and broken hairs were also noted. Dermoscopy revealed coiled corkscrew-shaped, comma-shaped, and zigzag hairs, along with scales, crusts, and black dots.
