CHCs have a frontline perspective on the impact of poverty, and they play a crucial role through the provision of comprehensive, wraparound care that addresses both health and social needs to alleviate the impacts of poverty and lack of affordability on the individuals and communities they serve. Learn more here: https://www.cachc.ca/affordability/

In June, we spent a day with Victorian Community Health CEOs exchanging ideas, getting inspired about ways we can collaborate globally, and are excited by ideas that can be implemented across the Canadian Community Health Centre landscape. It was great to hear that the delegation from VHA saw great learnings in the various Ontario CHCs they visited over the course of their time in the Toronto area. The VHA delegation was able to visit Niagara Falls Community Health Centre, Quest Community Health Centre, Flemingdon Health Centre and Centre Francophone du Grand Toronto.

The delegation also took part in the Alliance For Healthier Communities annual conference where they shared the Victorian health landscape with eager listeners and got great insight into other Canadian colleagues on how their CHCs operate and navigate the Canadian health landscape.

We are excited to share the CACHC 2026–2030 Strategic Plan: Healthy People, Healthy Communities.

Shaped by the voices of our members, partners, and the communities CHCs serve every day, this plan sets our direction for the next five years across three pillars: Organizational & Sector Stewardship, Network & Sector Building, and Policy & Systems Influence.

Together, these pillars will help move us toward a model of care that is accessible and equitable for everyone.

Read the full plan in English or French at the link below.

Take a look at these case studies to see real-world examples of how certain CHCs in Alberta, Manitoba, and Ontario deliver SRH education to unhoused and substance-using populations, create ad-hoc visuals to adapt SRH resources for diverse populations, and navigate the lack of physical abortion resources caused by stigma and community pressures.

This resource is designed to be printable and shareable online. We encourage you to share it with your peers and colleagues digitally or in the form of a pamphlet/booklet and include it on your organization’s website.
