Yesterday, warned Parliament about the dangers of expanding euthanasia to mental illness - and how it will also harm family members and friends of those whose lives are ended by euthanasia.

This recent exchange suggests there may be no clear requirement for assessors to know how many times a person has been denied. That opens the door to doctor shopping—until one approval is found.

When the outcome is irreversible, even small gaps in oversight should concern all of us.
