Yesterday, WOCRC’s team presented at the International Foundation for Integrated Care’s (IFIC) webinar on social prescribing. It was an incredible opportunity to share learnings from our social prescribing model, as well as our plans for the future, with an audience of nearly 200 participants from across the globe.
The team highlighted how WOCRC’s approach supports resident well-being by addressing the social determinants of health in partnership with local primary care clinics. They also shared research findings demonstrating the value social prescribing adds to the primary care model, including reduced hospital and emergency department admissions, and improvements in clients’ overall well-being.

Hier, l'équipe du CRCOO a présenté lors du webinaire de l' International Foundation for Integrated Care (IFIC) sur la prescription sociale. Il s'agissait d'une occasion exceptionnelle de partager les apprentissages tirés de notre modèle de prescription sociale, ainsi que nos orientations futures, avec un auditoire de près de 200 participants provenant du monde entier.
L'équipe a mis en évidence la façon dont l'approche du CRCOO soutient le bien-être des résidents en abordant les déterminants sociaux de la santé en partenariat avec des cliniques locales de soins primaires. Elle a également présenté des résultats de recherche démontrant la valeur ajoutée de la prescription sociale au modèle de soins primaires, notamment une réduction des admissions à l'hôpital et aux services d'urgence, ainsi qu'une amélioration du bien-être global des clients.
