Transforming Care for Indigenous Children Through Jordan’s Principle

Indigenous children and families continue to face systemic barriers in accessing safe, timely, and culturally appropriate health care.

At the Rehabilitation Centre for Children (RCC), meaningful change is underway. Guided by Jordan’s Principle and the Truth and Reconciliation Commission’s Calls to Action, RCC is working in partnership with First Nations communities to co-design care that reflects Indigenous values, knowledge, and priorities.

This approach centers Indigenous leadership, including a dedicated Wisdom Keeper, community partnerships, and active involvement from families, ensuring care is not only accessible but culturally safe and responsive.

The impact is significant: more than 6,000 children have accessed therapy services, many for the first time, within their own communities, reducing travel barriers and strengthening trust in care.

By embedding cultural safety and collaboration at every step, RCC is transforming health care experiences for Indigenous children and families. At the Centre de Réhabilitation Château de Colpach (CRCC), dedicated teams help patients rebuild their physical health, functional abilities, and psychological well-being.

The CRCC brings together two national centres of excellence: the National Post-Oncology Rehabilitation Service and the National Physical Rehabilitation Service. Through specialized care and personalized treatment plans, these programs help patients recover, regain independence, and improve their quality of life.

In June 2026, Accreditation Canada awarded the CRCC Platinum-level accreditation. This achievement recognizes the centre’s longstanding commitment to improving the quality and safety of care and delivering excellent services for every patient. Croix-Rouge luxembourgeoise

Le Centre de Réhabilitation du Château de Colpach (CRCC) accompagne ses patients dans leur rétablissement physique, physiologique et psychologique. Il abrite deux services nationaux d’excellence : le Service National de Réhabilitation Post-Oncologique et le Service National de Réhabilitation Physique. Grâce à une prise en charge spécialisée et à des programmes individualisés, ces services favorisent une récupération optimale des capacités fonctionnelles, le maintien de l’autonomie et l’amélioration de la qualité de vie des patients.

Improving Access to Syphilis Treatment in Emergency Departments Across Fraser Health (British Columbia)

During a provincial syphilis outbreak, timely and accurate treatment is critical, but for many individuals facing social and structural barriers, care is often delayed or missed altogether.

To address this, Fraser Health launched a cross-program quality improvement initiative to strengthen access to syphilis treatment within emergency departments as the primary point of care for many vulnerable populations.

By implementing standardized treatment pathways, EMR alerts, onsite medication packages, and targeted staff training, the initiative significantly improved treatment accuracy and access.

The results were transformative: medication administration accuracy increased from 27.5% to 92.5%, while empiric treatment more than doubled, ensuring more patients receive timely care when it matters most.

This initiative shows how coordinated, system-level improvements can enhance patient safety, reduce errors, and improve outcomes during a public health outbreak.

The North & East GTA Stroke Network coordinates stroke care across a large and diverse region as part of the Toronto Stroke Networks and supports patients across the full continuum of care, from emergency response and hyperacute treatment to rehabilitation and community reintegration.

Many of their sites have achieved Stroke Distinction, including Sunnybrook Health Sciences Centre, Scarborough Health Network, and St. John’s Rehab, highlighting the high quality of care across the region and a shared commitment to best practices.

Sunnybrook’s Bayview site serves as the regional acute comprehensive stroke centre, Scarborough Health Network provides acute stroke and inpatient rehabilitation, and St. John’s Rehab delivers specialized rehabilitation services.

Improving Access to Care Through Digital Connection in British Columbia

Access to health care shouldn’t depend on having a phone — yet for many people, it does. The Test, Link, Call (TLC) program, implemented by the British Columbia Centre for Disease Control, is transforming access to care for people experiencing criminalization and other structural barriers. By providing free smartphones, six-month talk & text plans, and optional peer mentor support, TLC is helping individuals stay connected to essential HIV, hepatitis, and syphilis care.

Launched in 2021 to support people leaving correctional facilities, TLC has since expanded province-wide, reaching over 1,000 clients and partnering with more than 25 clinical services.

By addressing digital access as a key determinant of health, TLC is reducing missed appointments, increasing treatment uptake, and building trust with historically underserved populations. Learn more about this Leading Practice: https://healthstandards.org/leading-practice/test-link-call-project/
