Our 'Health Made Simple' info session last week was a great success!

Thank you to Mosaic Home Care & Community Resource Centre for hosting and to our partners, North York General Hospital, Flemingdon Health Centre, Better Living Health and Community Services and Get Well Health Team for your support.

We're grateful for the opportunity to connect with community members and share information on how to navigate health care information, local health care and support services, chronic disease management and the importance of being connected to primary care.

Last week, we hosted our fourth annual Connecting Care Evening, with the generous support from North York General Hospital Foundation, Layla and the Ontario Medical Association.

130 primary care, specialist and community colleagues came together to network and learn more about how their is working on creating a more integrated health system and the opportunities to get involved.

Thank you to our primary care and specialist colleagues, hospital leadership, patient and caregiver council members and OHT partners for attending!

With the support of our attendees, we also collected nearly 100 lbs of food donations for North York Harvest Food Bank!

We held our first Community Health & Information Fair ( ) at Yonge Sheppard Centre on September 11.

More than 80 community members came by, and we provided 67 chronic disease risk assessments. Services were free and people had the opportunity to pick up helpful resources and information on health care, community services, cancer screening, banking for newcomers and more.
