Today is National Advance Care Planning Day ! We encourage you to spend some time to sit down with your family and/or friends to talk about Advance Care Planning (what is important to you, who will speak for you if you are unable to speak for yourself, choose a Substitute Decision Maker and make sure they know what your wishes are, etc.).
Advance Care Planning discussions should happen often, along with updating important documents (Power of Attorney for Personal Care, Power of Attorney for Property, Insurance Policy Beneficiaries and Wills) reviewed regularly, especially if there have been any significant life changes (ex: new family members born, relationship changes, death in family, acquire/dispose of major assets, etc.).
