Meet Chelsea Bartja, Forensic Social Worker at the C.K. Clarke Centre, our Transitional Rehabilitative Housing Program.

Chelsea and the Clarke team support individuals in the forensic mental health care system through providing psychosocial rehabilitation, developing life skills, addressing and managing addiction-related challenges, and making connections within the community.

“My ‘why’ is that I want to contribute a little joy and hope to people’s stories. People make sense of their lives through the stories that are told to them, the stories that are told about them, and the stories they tell about themselves.”

Her work at St. Leonard's centres around helping others take ownership of their stories and see themselves in a different light, creating opportunities for hope, growth, and connection.

From 2025 to 2026, our Registered Practical Nurses (RPNs) provided more than 1,400 visits to participants facing challenges related to mental health, addiction, chronic health conditions, and homelessness.

Their work focuses on building trust, meeting people where they are, and providing compassionate, non-judgmental care that prioritizes dignity, autonomy, and safety.

Funded independently of the City, our RPNs help participants reconnect with medical supports by creating more positive, respectful experiences and reducing barriers to care.

Join our SIL team and help adults with developmental disabilities, mental health needs, and/or addictions build the skills, confidence, and connections they need to live independently in the community.

In this role, you'll work closely with participants to develop goals, build life skills, connect to resources, and navigate challenges, while supporting their wellbeing, independence, and long-term success.

✔ Support individuals in achieving their goals and living independently

✔ Connect participants to community resources and support services

🧡 In recognition of the National Day for Truth and Reconciliation, members of the SLCS team visited Ska-Nah-Doht Village last week to learn more about Indigenous history, culture, and traditions.

Our visit provided a meaningful opportunity to learn about Indigenous communities and ways of life from more than 1,000 years ago. The village features a recreation of a Haudenosaunee longhouse community, reflecting one of the many Indigenous Nations that have called this land home.

At SLCS, we recognize that reconciliation is an ongoing journey. Experiences like this help deepen our understanding and remind us of the importance of listening, learning, and building meaningful relationships with Indigenous communities.

🧡 Today, staff across SLCS are wearing orange shirts in recognition of the National Day for Truth and Reconciliation.

We honour the Survivors of residential schools, remember the children who never came home, and recognize the ongoing impacts of residential schools on Indigenous families and communities.

Reconciliation is an ongoing commitment to learning, reflection, and action. Today, and every day, we encourage our community to listen, learn, and take meaningful steps toward reconciliation.

This year, we ordered shirts with art by Jay Joseph from Indigenous Marketing. Thank you for all the work you do!

We wrapped up the summer with our annual BBQ, and it was a great way to celebrate the season together!

From burgers fresh off the grill by our amazing team members to cotton candy, a piñata, and face painting, there was plenty of fun to go around. 🍭🎉🎨

With good food, lots of laughs, and great company, it was a wonderful afternoon spent connecting and enjoying the last days of summer. 💛

Our BBQs seem to get a little better every year, and we’re already looking forward to next summer’s celebration

A heartfelt thank you to Keagan and the team at Walmart Supercentre #0349 – Argyle Mall for providing a Community Grant in support of SLCS’s C.K. Clarke Centre. C.K. Clarke Centre is a six-bed residence providing 24-hour support to adults transitioning from the forensic mental-health care system.

Walmart’s generous grant will help make the residence feel more welcoming and more like home. Funds will support new pillows and bedding, lamps, rugs, shelving, plants, paint and other thoughtful touches.

A new pillow, a comfortable room or a space that feels like your own may seem simple—but these everyday comforts can offer dignity, belonging and a sense of stability during an important transition.

Thank you to our neighbours at Walmart Supercentre #0349 for recognizing that a supportive environment can help create the foundation for another opportunity and a more hopeful future.

🏠 Over the past two years, 82% of reintegration referrals involved people with no fixed address. This statistic highlights the strong connection between housing instability and the challenges many individuals face when returning to the community.

Our Reintegration Program helps bridge that gap by connecting people to housing, supports, and resources that promote stability and success.

Since 2023, Lynn's Bakery has generously donated baked goods to St. Leonard's Community Services every summer, helping us support participants across multiple programs throughout our agency.

These donations help us respond to immediate needs, provide comfort and encouragement during difficult times, and celebrate the accomplishments of the individuals we serve. Something as simple as a fresh baked treat can spark connection, boost motivation, and show someone that their community cares.

We are incredibly grateful to AJ and the Lynn's Bakery team for four years of partnership and generosity.
