Meet Maryam Ojo, Quality Improvement Decision Support Specialist

"A big part of what I do is translate complex health and EMR (Electronic Medical Record) data into meaningful insights that support quality improvement initiatives, identify opportunities for clinical process change, and inform decision-making."

Yesterday afternoon, we had the pleasure of hosting Hamilton Mountain MPP Monica Ciriello on a tour of two of our family practice sites in her riding: Queensdale Hub & Clinic and Fletcher Medical Centre.

She was able to meet our incredible practice teams, see the work in action, and hear about the benefit to patients of having a team of healthcare professionals working together with family doctors and nurse practitioners to provide care.

Thank you to MPP Ciriello for taking the time to recognize our teams and see firsthand the impact of Ontario's historic investment into primary care. We'd like to extend our congratulations to Sukhpaul (Paul) Tut, our new Chair of our Board of Directors! 🎉

Paul is a longstanding community member, lawyer, and HWDSB Board of Trustees Vice-Chair and Trustee for Dundas and West Flamborough. He is passionate about helping to create a healthy and connected community; it is with this lens that Paul has served on the HFHT Board of Directors since 2020, as Vice-Chair since 2024, and will continue to serve in his new role of Chair.

We are pleased to announce the appointment of Dr. Jason Aguanno as Lead Physician, effective Monday, October 5th, 2026.

Dr. Aguanno has served the Hamilton community for over two decades and has been part of the HFHT since its inception. A widely respected clinician and leader, he most recently served as Medical Director of the Escarpment Health Centre, where he spearheaded the co-location of seven practices into an integrated Health Home model. He led Unattached Patient Clinic initiatives and was successful in attaching patients into the newly co-located practice for longitudinal care.

His leadership background also includes nine years as Lead Physician of the Hamilton Escarpment Family Health Organization (FHO), two tenures as HFHT Associate Lead Physician, and continuous service as an Assistant Clinical Professor in McMaster University’s Department of Family Medicine.

ADHD Awareness Month is observed annually in October across Canada, aligning with global efforts to increase understanding of ADHD, reduce stigma, and provide resources for individuals, families, and professionals.

Adults living with attention deficit hyperactivity disorder (ADHD) or wondering whether they might have it, may often navigate challenges that can feel confusing, overwhelming, or isolating. You are not alone.

Our free ADHD for Adults Workshop Series is led by an interprofessional team, offering clarity, community, and practical tools in a supportive environment.

Medicine comes in many forms. 🪶

That’s the message that Teresa Joudrey, owner of , a business that offers Indigenous-led cultural-based learning circles, wants to share.

“It’s not about replacing clinical medicine” she explains. “It’s about expanding our understanding of what healing can encompass. Stories themselves carry medicine.”

As we approach National Day for Truth and Reconciliation on September 30, we wanted to share a very special conversation we had with Teresa, who since last year has been facilitating the KAIROS Blanket Exercise for Hamilton Familly Health Team staff. We spoke about her journey starting Santele's Healing Circles, what to expect when doing a Blanket Exercise, how to approach medicine and healthcare through a decolonial lens, and how organizations and individuals can practice reconcili-action.

Our Quit Smoking group is a supportive program led by a Pharmacist and Certified Respiratory Educator for individuals who are interested in reducing or quitting smoking. This group is free and open to the public.

On August 28, Hamilton Family Health Team staff, physicians, and practice staff and their families gathered at Adventure Village for our annual Summer Social! ☀️

From mini-put to rock climbing, bumper cars and batting cages, it was wonderful to see team members from across our 1,000-person ecosystem and meet their families and loved ones. We had some fun in the sun, some friendly competition, and a lot of team-building during a beautiful Friday afternoon.

Thank you to our event planning team for their hard work putting together this opportunity for us to connect! 👏
