For over 26 years, Dawn Atwell has been an integral part of Crown Point Family Health Centre, and for the last 13, she has led the team as their practice manager.

When the previous manager of the clinic retired, the team believed in Dawn, even if she wasn’t sure of her own path. “Did I ever think I would be a manager? No. They helped nurture and grow that. They sent me to school to take some managerial courses,” she explains.

“I was shy and didn’t advocate much for myself. Now I do, for lots of people,” she smiles. “They saw in me something I didn’t see myself.”

Now, Dawn shares her unexpected path, managing the clinic like a home, expanding patient care, and the key to 26 years.

Last August, during Hamilton’s Art Crawl, community members stopped by the Hamilton Family Health Team (HFHT) booth and shared what health means to them through drawings and written reflections. 🎨🖊

One year later, those drawings are now displayed in one of HFHT’s waiting rooms. 

This is one of many examples of how HFHT's Patient and Family Advisory Group (PFG) members have helped bring ideas to life.

We are currently seeking patients to join and volunteer for the PFG. As a member, you will help inform decisions, provide feedback, and work alongside HFHT staff to improve the care experience for patients and families. Apply by August 31, 2026.

We are currently seeking HFHT patients to join our Patient and Family Advisory Group (PFG) to help us move forward in making decisions to better meet the needs of HFHT patients and families through patient-centred care.

This morning, we had the distinct pleasure of hosting the Honourable Sylvia Jones, Ontario's Deputy Premier and Minister of Health, and MPP Monica Ciriello for a tour of our small but mighty primary care clinic within the Eva Rothwell Centre. Read more: https://buff.ly/MSg4tXT

We celebrated our collective progress, in collaboration with our partners within the Greater Hamilton Health Network (GHHN) Primary Care Network, towards connecting all residents of the Greater Hamilton Area to a family doctor or nurse practitioner. We also had the opportunity to showcase the work in action: our clinic, Health Care at Eva Rothwell Centre, brings primary care to an established neighbourhood hub where individuals and families in Hamilton's North End can access lifechanging social supports and be part of a vibrant, welcoming community.

Thank you, Minister Jones and MPP Ciriello, for taking the time to visit and meet our teams, and for the Province's historic investments into primary care. We're excited to continue expanding our impact with the most recent investment of nearly $5 million to attach 10,400 more patients to primary care in the Greater Hamilton Area. With your help and in tandem with our partners, we are leading the way to a healthier community.

Thank you to our outgoing Chair of our Board, Dr. Jordan Wronzberg, who has dedicated 6 years to the Hamilton Family Health Team's Board of Directors and the past year as Board Chair. Jordan is an active family physician at the growing Carlisle Medical Centre, and has had a busy year caring for his patients, being a dad to his young family (including lots of athletic meets!), and actively teaching as an Assistant Clinical Professor in McMaster's School of Family Medicine. Jordan will continue on the Board as Past Chair for this coming year.

Meet Emily Despond, the Hub Coordinator for the Hamilton Family Health Team’s Queensdale Hub.

"I have always been driven by a strong desire to make a meaningful impact on the lives of people in my community. Early on, I realized that some of the most vital work in healthcare happens behind the scenes—ensuring that providers have what they need to work efficiently and that patients don’t feel lost in a complex system.

Combining my passion for helping others with my operational and organizational strengths brought me to the Hub Coordinator role. It allows me to be the supportive backbone for both health professionals and patients, which I find rewarding."

Meet Mary Dunn, a Respiratory Educator, helping patients breathe easier.

"I’ve been an RN for nearly 40 years and transitioned into Respiratory Education with the HFHT 15 years ago. Growing up with asthma has given me a deeper understanding of patients who struggle with their symptoms, often accepting them as a normal part of life."

"I conduct spirometry testing and provide education for patients with asthma or Chronic Obstructive Pulmonary Disease (COPD). Helping patients understand how their lung function is affected by their respiratory disease supports better symptom control through optimized treatment and meaningful lifestyle changes."

For Dr. Olawale Ayeni, empathy has always been at the heart of his work.

“People are always referring family members to me and asking if I’m taking on new patients, which is the sincerest form of flattery,” he says with a smile. “To have somebody say, will you take care of my wife, daughter, or my grandchildren is the biggest compliment you can get a family physician.”

But growing up, Dr. Ayeni didn’t always know he wanted to be a doctor.
