Housing that lasts takes support.
Housing someone is not the finish line. People also need the right supports to keep that housing.
Shelter was never designed to be a long-term housing solution. For many people, housing on its own is not enough. Too often, people cycle between shelter, hospitals, crisis services, justice systems, and back again.
People need support to stabilize, build skills, connect to care, prepare for housing, and stay housed after they move out. Horizons provides that support through life skills training, housing-focused planning, and follow-up after move-out. The model helps people not only get housed, but stay housed.
Since opening, 41 residents have moved into other housing options, and 88% of residents who transitioned out remain housed today. Residents who moved out have also maintained 10,772 nights of housing in the community.
