Who can speak for you if you can't?

Helping you plan ahead, one conversation at a time.

Part of Advance Care Planning is thinking about who you trust to make decisions on your behalf if you are unable to speak for yourself.

There are several ways your wishes can be supported, including a Representative Agreement, a Temporary Substitute Decision Maker, an Advance Directive, and an Enduring Power of Attorney.

Having these conversations and documents in place can provide peace of mind for you and your loved ones and help ensure your wishes are understood and respected.

What is a MOST Form?

Helping you plan ahead, one conversation at a time.
A MOST Form (Medical Orders for Scope of Treatment) is a document that helps communicate your wishes about medical care if you are unable to speak for yourself.

It can provide guidance to your medical team about the types of treatments and life-saving measures you would or would not want in an emergency or serious health situation.

These are not always easy conversations to have, but they can help ensure your wishes are understood and respected by your loved ones and healthcare providers.
One of the most important things to remember is that your MOST Form should be available to the people who may need it in a time of crisis, including family members, your doctor, and your medical file.

The BC Centre for Palliative Care and BC health authorities provide resources and examples of MOST Forms to help people better understand this process.
