Ready4Life The program targets youth age 16 and older who are at risk or vulnerable.

The Support offered is consistent with the founding program offered by SHH which is to provide affordable housing with supports to people with mental illness or mental health issues within the Halton community. Using a person-centred approach and a process of Recovery, this program achieves our primary core values - that individuals have the right to self-determination and access to inclusive, equ

itable services that empower them to integrate into the community and reach their highest potential. It is anticipated that the group may have one or more of the following issues: risk of homelessness, living in poverty, disconnected from family and school, mental health issues, minor involvement with the justice system and addictions. While every attempt will be made to intervene early in the referral process to reconnect youth to their family, education and youth services, we are specifically targeting those youth who have rejected, in the past, these essential supports and who are entering into, by their choice, adulthood.
