Do you have a passion for helping older people live independently in their own homes? Join PAWA Community Care Inc, a respected not-for-profit organisation with over 50 years of experience supporting multicultural communities across Sydney.
We're seeking an experienced Care Partner to work with our Support at Home (SAH) and Commonwealth Home Support Programme (CHSP) participants.
✅ What You'll Do:
• Conduct participant assessments and onboarding
• Develop and review person-centred care plans
• Manage individual budgets and funding reviews
• Coordinate services with allied health and community providers
• Support participants and families to achieve the best outcomes
• Promote independence, wellbeing, and reablement
• Work within a supportive multicultural care management team
✅ What We're Looking For:
• Experience in Aged Care, Disability, Care Coordination, or Case Management
• Diploma of Community Services (Case Management), Cert IV Aged Care, Disability Care, or similar
• Strong communication, organisation, and budgeting skills
• Knowledge of Aged Care Quality Standards, SAH and CHSP programs
• Current Police Check, First Aid Certificate, and Australian work rights
• Driver's licence and willingness to travel across Greater Sydney
⭐ Polish or other Slavic language skills are highly desirable!
🏠 Hybrid work arrangements available after probation.
