MIFWA - supporting mental wellness and recovery MIFWA is an independent community-based mental health organisation supporting people who are living with and recovering from mental health challenges.

We also support their families, carers, friends and colleagues who are supporting them. MIFWA is an independent community based mental health organisation with 30 years’ experience supporting people with a lived experience of mental health challenges, and their families and carers across Western Australia. We pride ourselves on being welcoming, accessible and approachable, and offer a warm and friendly environment where people can learn, contribute, get advice and assistance.

What else happens when support ends at the hospital door?

For many families, the peer support through the Hospital to Home program has been a lifeline, as it helps ease pressure on family members and carers, improves safety and stability at home, and supports better outcomes for everyone involved when a loved one is transitioning from hospital to home.

But if the Hospital to Home program closes, many will be left to fill the gap, often supporting their loved one without the preparation, resources, guidance or ongoing support they need.

So the impact can be significant for families:

❌ Increased emotional strain
❌ Increased financial strain
❌ Burnout
❌ Ongoing stress
❌Seeing their loved ones go through cycles of crisis that can lead to hospital re-admission

When Hospital to Home supports people after discharge, the peers also support those who stand beside them every day.

Today, on Thank a First Responder Day (10 June), we extend our heartfelt gratitude to the first responders who work tirelessly throughout our communities.

Many first responders support people arriving at Emergency Departments during some of the most challenging moments of their lives, including times of significant distress or psychosis.

They do this with care, compassion, empathy, and professionalism, at times also needing to support families through unimaginable grief.

While their focus is always on helping others, we know the work can take a toll on them. Today, we acknowledge their dedication, resilience and humanity.

Our partner hospitals tell us that without the Hospital to Home program there would be a significant gap in post-discharge support. For example, this team highlights that its absence would impact the progress patients could make during their inpatient admission, increasing the risk of setbacks and reducing the long-term benefits of their treatment.

Hospital to Home helps bridge the gap between hospital and community, giving patients the support they need to maintain their recovery, build independence, and achieve better outcomes:

"Peer support plays a vital role during the transition from hospital to home. It helps reduce feelings of isolation, increases confidence in managing mental health in the community, and supports continuity of care following discharge. Many patients identify more strongly with peer workers, which enhances their sense of hope, empowerment, and belief in their own recovery."

We're grateful for the lived experience-led organisations and advocates expressing deep concern about the uncertainty surrounding the future of the Hospital to Home program, and highlighting what's at stake if this vital support is allowed to lapse.

🗣"We see every day how critical the transition from hospital back into the community is. For many of the people and families we support, this period is not simply vulnerable - it is the point at which the system is most likely to lose them. They are too often unable to access other services because they are perceived as 'complex' or 'too hard.'

These labels - which reflect system limitations, not individual failings - frequently result in people being turned away, placed on long waitlists, or offered support that does not meet their needs.

For these communities, peer-led support is not an optional enhancement. It is often the only form of support that is trusted, accessible, culturally safe, and effective."

"Access to peer support upon discharge has greatly improved patients' mental health journeys. The opportunity to connect with someone who has lived experience of mental health challenges provides a level of understanding, validation, and reassurance that is difficult to replicate through clinical support alone. Patients consistently demonstrate stronger engagement and trust when support is offered by peers who genuinely understand the complexities of recovery and reintegration into everyday life."

In less than 30 days, our Hospital to Home program will be forced to close, leaving people being discharged without this life-changing support that could mean "the difference between making it through or not."

As a Lived Experience Advocate shared with us, this support matters. It helps stop the cycles of crisis and reduce hospital admission. It also helps prevent putting the emotional, mental and financial burden on the families and carers who will likely become the default providers of care without preparation or backup themselves.

Please support our petition so we can urge the government to provide emergency funding to help keep the program open.

Participants from MIFWA’s Youth Programs attended a Bowling and Pizza Group at Rosemount Bowls in celebration of Challenging Stigma Week.

The group was filled with laughter and camaraderie, and hidden talents were revealed when multiple young people scored strikes!

Here is what some of the attendees said they enjoyed about the group:

"Gaining friends, enjoying myself and meeting new people with similar interests or issues as me."

"Being around people of similar ages, actively participating in creative and engaging work (painting, bowling)."
