Our Story
The Collaborative Nurse’s inception came from a passion for wanting to help our elderly and those living with a disability in the community. We want to enable them to remain as independent as possible at home with the support of collaborative services. We are very passionate about what we do knowing we can help even in just the smallest of ways. We are dedicated to preserving dignity, choice and decision making, giving and respect and maintaining each person’s integrity.
The Collaborative Nurse offers Brokerage services and Fee for Service for Private Clients. We can also support Self-Managed clients on Home Care Packages or NDIS.
We have a range of services including but not limited to:
*Personal Care, Social Support, Domestic Services, Meal Prep, Escorting/Accompanying to Appointments, Assistance with Shopping, Welfare Checks, Medication Prompts.
