Short-term care pathways are the different temporary support options older people can access when they need extra help for a short time through My Aged Care.

1- Short-Term Restorative Care (STRC) – Up to 8 weeks of support to help older people regain strength and independence through physio, OT, exercise, and daily living support, aiming to improve function and ability to stay at home.
2- Transition Care Programme (TCP) – Up to 12 weeks of short-term care after a hospital stay, either at home or in a facility, providing rehab, nursing, and daily support to help recovery and avoid long-term residential care.
3- End-of-Life Care (EOL) – Usually up to 12 weeks (depending on need) for people in the final stage of life, focused on comfort, dignity, and quality of life, including nursing care, personal care, equipment, and palliative support, often delivered at home.

Many elderly people don’t realise the kind of support available through My Aged Care. It’s not just for when things become very difficult — help can come in simple, everyday ways like support with cleaning, shopping, transport, small home modifications for safety, meal preparation or delivery and even social support to stay connected.
Sometimes just knowing these options exist can make life so much easier and help people stay independent for longer.
