NVC Group is looking for experienced and motivated Home & Community Care Coordinators to join our Home and Community Care team in Coffs Harbour and Kempsey.
These full-time roles involve coordinating member services, assisting with assessments and care plan development, supporting personalised budgets, and liaising with members, families and health professionals to ensure high-quality care.
This is a great opportunity for someone with strong communication, coordination and community care experience to join a supportive, community-owned not-for-profit organisation.
