How We Can Help
We work in partnership with families, carers, child safety, stakeholders and the young person themselves to identify the most suitable residential care placement, which will fully meet their requirements and provide the best level of support. Our residential services focus on empowering young people to develop their social, emotional, independence and functional skills, enabling them to acquire coping mechanisms for the future, and take steps towards resilience and wellbeing.

Community Support Worker

Support NDIS participants to live independently and achieve their personal goals through:

* Community access and social participation
* Personal care (where required)
* Daily living support
* Transport to appointments and activities
* Skill development and independence
* Accurate documentation and case notes

Community Development Officer

Help strengthen community connections by:

* Developing and coordinating community programs and events
* Building partnerships with local organisations and stakeholders
* Creating opportunities for social inclusion and community participation
* Promoting Pathfinder Care services throughout the community
* Identifying community needs and developing innovative programs
* Assisting participants to access local services, education, employment and volunteering opportunities

Housing Officer

Support participants with safe and sustainable housing by:

* Assisting participants to secure and maintain suitable accommodation
* Liaising with housing providers, landlords and support agencies
* Conducting property visits and wellbeing checks
* Supporting tenancy management and housing-related goals
* Assisting with housing applications and advocacy
* Helping participants develop independent living skills

What We’re Looking For

We’re seeking people who are:

* Compassionate, reliable and professional
* Excellent communicators with strong interpersonal skills
* Passionate about supporting people and strengthening communities
* Able to work independently and within a team
* Flexible and committed to delivering high-quality supports

Essential Requirements

* Current Australian Driver Licence
* Reliable, fully insured vehicle
* Current NDIS Worker Screening Check (or willingness to obtain)
* Working with Vulnerable People Registration
* Current First Aid & CPR Certificate
* National Police Check
* NDIS Worker Orientation Module Certificate
* Excellent written and verbal communication skills
* Strong computer and organisational skills
* Full Australian working rights

Our crew was at it again on Friday, and as always, the Pathfinder Care workshop was full of laughs, friendly competition and plenty of smiles during bingo.
Seeing everyone having so much fun, making new memories and cheering each other on has us thinking, we might just have to make bingo a regular thing.🥰
Days like these remind us that it’s not just about the game, it’s about connection, inclusion and creating moments that everyone can enjoy together.

Four years ago, Scott’s life changed forever after a devastating motorcycle accident. The road to recovery hasn’t been easy, but with determination, the right support, and an incredible team around him, Scott continues to achieve things many thought might never be possible.

A huge thank you to Dan and the team at In Balance Physio for the outstanding work you’ve done throughout Scott’s rehabilitation. Your knowledge, encouragement, and commitment have played such an important role in helping Scott regain his strength, confidence, and independence.

This short video is just a small glimpse into how far Scott has come. His journey is a testament to what can be achieved through hard work, persistence, and the support of passionate professionals.

Job Description: We are seeking a compassionate and dedicated Community Worker to join our team.
In this role, you will provide support and assistance to individuals in their homes and the community, helping them to achieve their goals and maintain their independence.
Key Responsibilities:
* Provide personal care and support to clients, including assistance with daily living activities.
* Assist clients with mobility and transportation to appointments and social activities.
* Support clients in developing and maintaining social connections.
* Implement care plans and follow instructions from healthcare professionals.
* Maintain accurate records and documentation of care provided.
* Communicate effectively with clients, their families, and other team members.
Requirements:
* Relevant qualifications in community services, aged care, or disability support.
* Previous experience in a similar role is preferred.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Valid driver's license and reliable transportation.
* Current First Aid and CPR certification.
* National Police Clearance and Working with Children Check (or willingness to obtain).
