Job Description: We are seeking a compassionate and dedicated Community Worker to join our team.
In this role, you will provide support and assistance to individuals in their homes and the community, helping them to achieve their goals and maintain their independence.
Key Responsibilities:
* Provide personal care and support to clients, including assistance with daily living activities.
* Assist clients with mobility and transportation to appointments and social activities.
* Support clients in developing and maintaining social connections.
* Implement care plans and follow instructions from healthcare professionals.
* Maintain accurate records and documentation of care provided.
* Communicate effectively with clients, their families, and other team members.
Requirements:
* Relevant qualifications in community services, aged care, or disability support.
* Previous experience in a similar role is preferred.
* Excellent communication and interpersonal skills.
* Ability to work independently and as part of a team.
* Valid driver's license and reliable transportation.
* Current First Aid and CPR certification.
* National Police Clearance and Working with Children Check (or willingness to obtain).
Visa Application Standards:
• This position is suitable for individuals on visa that allows for full time employment.
* Applicants must meet the visa requirements for working in Australia.

Today was Steff’s last day with Pathfinder Care, and her going away lunch was definitely a bittersweet moment for all of us.

Steff has been part of the Pathfinder family and has shared many moments with our team and the people we support. While it’s always sad to see someone move on, we truly wish her all the very best in her new chapter at Mayshaw.

We hope this new opportunity brings her happiness, success, and everything she is looking for.

Whoever fixed our garden post in the Pathfinder Care car park is an absolute legend! 🙌

We noticed the garden post had been repaired, but unfortunately we couldn’t find the camera recording as we’re not sure exactly when it was done. Whoever took the time to fix it without saying anything, we truly appreciate it.

Our garden edging in the car park was starting to break apart, and seeing someone step in and quietly repair it means a lot to us and the whole Pathfinder Care family.

Small acts of kindness like this don’t go unnoticed.

🖤🤍 Pathfinder Care Proudly Supporting Community Inclusion Through Sport 🤍🖤
We are incredibly proud to see our community partnership featured in this week’s news highlighting Luke Fish’s footy program at the Scottsdale Football Club.
At Pathfinder Care, we believe inclusion is more than just a word, it’s action. This season, Leigh Barnes, Mitchell Brown, Cameron Hartley and Ella Faulkner stepping into exciting new roles as apprentices, learning the ropes from local legend Luke Fish and gaining hands-on experience behind the scenes at the club.
Country footy has always been about connection, pride and community spirit and we’re honoured to support opportunities that build confidence, friendships and meaningful participation for our participants.
Sport brings people together. When everyone feels welcome, included and valued, our whole community becomes stronger.
A big thank you to Luke Fish, the Scottsdale Football Club, and everyone involved for embracing genuine inclusion and creating pathways for growth.
We can’t wait to see what this season brings! 🏉👏

The team at Al’s Barbershop are always incredibly accommodating and truly understand the sensory needs of each and every participant from Pathfinder Care. That level of patience, awareness, and professionalism makes all the difference.
